Job Description
Advanced Clinical Pharmacist - General Medical Wards (Urgent Care) Countess of Chester Hospital NHS Foundation Trust

Working Hours: Up to 37.5 hours (part-time and flexible working will be considered).

Contract: 12-month fixed term contract.

An opportunity has arisen for an enthusiastic, committed and self-motivated pharmacist to join our team, providing an advanced clinical pharmacy service to the Urgent Care Division.

Our friendly department supports innovation, learning, and teamwork, encouraging individuals to develop in their role. We aim to improve patient care and promote professional development. We are a forward-thinking department using robotic dispensing, electronic prescribing, and electronic pharmaceutical care planning. To meet patient needs, we provide ward-based clinical pharmacy, pharmacist-led clinics, and have an extensive team of independent prescribers.

Applicants should ideally have, or be prepared to work towards becoming an independent prescribing practitioner to manage medication throughout the patient's stay and ensure a high quality and efficient pharmacy service.

Applicants should hold a postgraduate diploma in clinical pharmacy, or demonstrable equivalent experience (see person specification), and must be registered as pharmacists with the GPhC.

Main Duties of the Job
  1. Work with the pharmacy clinical service managers to identify, plan, and develop new opportunities within the Urgent Care division.
  2. Apply expert knowledge to undertake an advanced clinical pharmacist role in the provision & development of pharmaceutical services to patients under the care of medical teams within the urgent care division.
  3. Review discharge processes to maintain and improve patient flow.
  4. Provide expert advice and guidance on the safe and effective use of medicines for patients, carers, and other healthcare professionals within the urgent care division.
  5. Provide education on aspects of pharmaceutical care for pharmacy staff and other staff disciplines (e.g., nurses, junior doctors, PAM's).
  6. Monitor compliance with NICE guidance and TAs, and other locally agreed guidelines and pathways relating to the urgent care division.
  7. Assist in the development of policies, guidelines, and pathways relating to drug therapies used within the urgent care division.
  8. Where appropriate, prescribe medication as a non-medical prescriber, within areas of expertise, in accordance with a Trust-approved personal formulary.
  9. Where appropriate, work as an independent practitioner, examining patients & developing a management plan, in accordance with Trust approval.

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations:

  • The Countess of Chester Hospital: providing 438 general and acute beds
  • Ellesmere Port Hospital: providing 56 beds as a rehabilitation, intermediate and outpatient facility
  • Tarporley War Memorial Hospital: a base for community services which serve the local rural population.

Person Specification Qualifications
  • Vocational Masters Degree in Pharmacy
  • Postgraduate clinical pharmacy diploma or demonstrable equivalent NHS hospital experience
  • Registration with the General Pharmaceutical Council
  • Evidence of ongoing continued professional development
  • Independent prescribing qualification or commitment to achieving following appointment
  • Clinical examination skills
Knowledge and Experience
  • Specialist knowledge within the area of practice
  • Delivery of clinical pharmacy services to a range of specialities
  • Experience of teaching or tutoring
  • Experience of audit, practice research or projects
