Opportunity available for an Advanced Nurse/Clinical Practitioner to work within an Urgent Care Centre across the West Midlands.
The team sits within an Urgent Care Centre where the role will predominantly be responsible for the delivery of evidence-based nursing services. You will work as part of the multi-disciplinary team, delivering care within your scope of practice to the entitled patient population.
As a Nurse Prescriber, you will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, and well man clinics. You will also support the management team in reviewing clinical policy and procedure. Access to appropriate clinical supervision and a designated individual in the organisation will be provided for general advice and support on a day-to-day basis.
In this role, you will develop, implement, and embed health promotion and wellbeing programmes, as well as evaluate individual treatment plans for both acute and non-acute conditions. You will support the clinical team in developing individual treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions to prevent adverse effects on their health.
Providing routine nursing care to patients in accordance with clinical evidence, NICE, and NSF is essential. You will also be able to prescribe medication in accordance with PGD and within your scope of practice, reviewing medication for effectiveness, need, and national guidance.
