Advanced Clinical Practitioner Remote Clinical Assessment

Team SWASFT are seeking highly motivated ACPs with an appetite for remote assessment and driving quality to join our team within our Emergency Operations Centres. Following the successful roll out of our sessional / bank ACPs we are now looking to increase ACP cover across the full working week.
The role is designed to allow an 90/10 clinical split , fully supporting you in developing within all four pillars of Advanced Practice with built in supporting professional activity time.
The key ambition of the role is to support SWASFT in achieving its ambitious Hear and Treat agenda whilst driving quality across the team.
Main duties of the role include:
- Telephone consultations directly with patients, relatives and/or their carers.
- Reviewing, triaging and reprioritising of 999 calls.
- Closing incidents without deploying an ambulance resource.
- Provision of senior clinical advice to EOC and patient facing colleagues.
- Senior clinical visibility.
- Clinical audit and case review.
- Education and peer supervision.
ACPs must hold a full MSc Advanced Clinical Practice or equivalent (NHSE Advanced Practice Digital Badge) and have at least 2 years recent telephone triage experience within primary care, out of hours or ambulance service. Full training and development will be provided to support you in your role.
The core shift timings are and can be completed from our Emergency Operations Centres in Bristol or Exeter.
Main duties of the job- To fulfil the expectations and core capabilities of an AP as set out in the Trust AP job description.
- To work autonomously at an advanced level providing high quality patient care.
- Utilise the four pillars of advanced practice to support delivery of the Trust objectives in keeping with the Trust values.
- To provide remote clinical assessment for patients and their families / carers when presenting to the ambulance service.
- To manage patients presenting to the ambulance service completing full episodes of care through hear and treat or referring to alternative services as required (e.g. ambulance face to face, ED or GP).
- To provide senior clinical support, supervision and clinical validation for operational colleagues, for instance registered clinicians (nurse / Paramedic), Technicians and Community Responders.
- To provide senior clinical visibility for Emergency Operations Centre colleagues and trainee or qualified Specialist Practitioners in Urgent and Emergency Care.
- To promote the values of Advanced Clinical Practice throughout the Trust.

You will have access to bi-monthly team meetings and twice-yearly training mornings, in addition to yearly development days.
We currently have an opportunity for 1 x 30 hour, and 1 x 22.5 hour contract and/or the option to work a set day(s) each week. Please include your preference within your supporting statement. Rota details included within the supporting information section. Please note we are not seeking to recruit into bank roles currently.
Where you are recruited into a rota pattern of 18.75 hours per week.
Person Specification Education and Qualifications- Statutory Professional Registration with the HCPC, NMC or GPhC
- Full Masters Degree in Advanced Practice or equivalent (i.e. NHSE Advanced Practice Digital Badge)
- Independent non-medical prescribing qualification
- Full Driving License
- Level 7 Remote Clinical Decision Making Module
- Diploma in Primary and Urgent Care
- No less than two years experience delivering telephone triage without the use of clinical decision making software within primary care, Clinical Assessment Service or 111/OOH
- Providing remote clinical advice and support to junior colleagues
- Minimum of 5 years post registration experience with a contemporary clinical practice portfolio
- Evidence of practice in line with the HEE (2017) multi-professional framework
- Experience of delivering multi-professional supervision
