Primary Care Knowsley is a healthcare organisation based in Knowsley, dedicated to delivering and supporting high-quality primary care services across the borough. It is a GP Federation, meaning it is a collaboration of multiple general practices working together to improve patient care, streamline services, and support local health priorities.
The Federation often runs enhanced access services, such as evening and weekend appointments, to improve availability and convenience for patients.
Primary Care Knowsley works closely with the local Integrated Care Board (ICB), public health teams, and other NHS partners to address the specific health needs of Knowsley's population, such as high levels of deprivation, long-term conditions, and health inequalities.
Job responsibilitiesWe are seeking a highly skilled and autonomous Advanced Nurse Practitioner to join our dynamic and progressive team. The ANP will deliver high-quality, evidence-based clinical care to patients across a wide range of acute, chronic, and complex conditions. Working within a multi-disciplinary team, the ANP will be responsible for assessing, diagnosing, treating, and managing patients within the scope of their advanced practice.
The successful candidate will be an independent prescriber and able to work both independently and collaboratively to meet the healthcare needs of our diverse population. This role is essential to supporting continuity of care, improving access, and enhancing patient experience across our network of practices.
Person Specification Qualifications- The successful candidate will be a registered nurse with current NMC registration and will hold a Masters level qualification in Advanced Clinical Practice (or be working towards this), along with an independent prescribing qualification (V300).
- Significant post-registration clinical experience in a relevant setting (e.g., primary care, urgent care, community services).
- Demonstrated ability to assess, diagnose, treat and manage patients with a range of acute, chronic, and complex conditions.
- Advanced clinical assessment and decision-making skills.
- Ability to work autonomously within scope of practice.
- Strong understanding of evidence-based practice, clinical governance, and current NHS policies and guidelines.
- Confident in prescribing safely and appropriately in line with national standards.
- Competent in ordering, interpreting and acting on diagnostic tests (e.g., bloods, ECGs, X-rays).
- Excellent communication and interpersonal skills across all levels of staff and patient populations.
- Ability to work effectively as part of a multidisciplinary team and contribute to collaborative care.
- Competent in the use of clinical systems (e.g. EMIS Web, Docman) and maintaining accurate, timely clinical records.
- High level of professionalism, integrity, and accountability.
- Strong commitment to patient-centred care, clinical excellence, and continuous professional development.
- Understanding of CQC requirements, clinical risk assessment, and regulatory frameworks relevant to primary care.
- Previous experience working within general practice or Primary Care Networks (PCNs).
- Experience in urgent care, home visits, or out-of-hours services.
- Exposure to and confidence in managing mental health presentations.
- Involvement in clinical audits, research, or quality improvement initiatives.
- Experience in supervising, mentoring, or developing junior staff or students.
- Ability to support service redesign or development in response to changing patient needs.
- Awareness of digital health tools or remote consultation platforms (e.g., AccuRx, AskFirst).
- Familiarity with local pathways, referral systems, and working with multi-agency teams (e.g., social care, mental health services).
- Understanding of population health, health inequalities, and personalised care models.
- Flexible and able to adapt to the evolving demands of primary care transformation.
- Experience in managing long-term conditions such as diabetes, asthma, COPD, and hypertension.
