Salary: £52,000- £65,000 (Dependent on experience)

My client is seeking a highly skilled and motivated Advanced Nurse Practitioner to join their dynamic team at a reputable GP surgery.

Job Description:

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  1. Develop, implement and embed health promotion and wellbeing programmes
  2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  3. Implement and evaluate individual specialised treatment plans for chronic disease patients
  4. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
  5. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  6. Request pathology services as necessary
  7. Process and interpret pathology and other test results as required
  8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  9. Maintain accurate clinical records
  10. Ensure SNOMED CT codes are used effectively
  11. Maintain chronic disease registers
  12. Chaperone patients where necessary
  13. Prioritise health issues and intervene appropriately
  14. Support the team in dealing with clinical emergencies
  15. Recognise, assess and refer patients presenting with mental health needs
  16. Implement vaccination programmes for adults and children
  17. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
  18. Contribute to practice targets (QOF, etc.), complying with local and regional guidance
  19. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
  20. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
  21. Support the clinical team with all safeguarding matters in accordance with local and national policies
  22. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  23. Deliver opportunistic health promotion where appropriate
  24. Provide supervision and support to other members of staff such as Healthcare Professionals, Practice Nurses, Nurse Associates and Nurse Practitioners
  25. Attend clinical, MDT and practice meetings
  26. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels
  27. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner
