Salary: £38,244 to £39,202 pro rata
This is an exciting opportunity for the right candidate to be part of this dynamic and forward-thinking team. You will be mentored to develop advanced clinical skills to undertake specialist assessments and interventions with patients in the community who have a life-limiting illness.
The post holder should have experience within palliative care and be keen to develop advanced clinical skills. You will work within our community team to support the development of Single Point of Access and Virtual Ward projects. Following the addition of onsite clinics at Trinity Hospice, working within the Living Well services is also a key part of the role to deliver hospice support to patients and their loved ones following their palliative diagnosis.
Delivery of education is a key part of the role to enable acute and community colleagues to support their patients to 'live well'. You will work collaboratively within the Palliative Care Networks across the area to support the strategic vision of care needs.
You will be a team player with the ability to work autonomously and possess a willingness and motivation to make a difference. The post holder will rotate to support all Trinity services according to patient need.
The service is provided over 7 days. A UK driving licence and access to a car is essential.
