Advanced Nurse Practitioner - Community Mental Health Team

The service offers support and treatment to service users experiencing moderate to severe mental illness, in a community setting.

Role requires full Non-medical prescribing/ ANP qualifications.

Main duties of the job

The post holder will work as part of a multi-disciplinary team delivering care to service users in a range of community settings. Other members of the team include Consultant Psychiatrists, Hub Manager, Clinical Psychologists, Clinical Leads, Assistant Psychologist, Mental Health Nurses, Occupational Therapist, Well-Being Practitioner, Art Therapist, Medical Support Workers and STR workers. The ability to work flexibly in multidisciplinary teams and in a range of settings is therefore essential.

The post holder will, in conjunction with the management team, provide strong clinical leadership and expertise to the multidisciplinary team, ensuring that high standards are maintained.

The Advanced Nurse Practitioner will hold a caseload and will manage regular follow-up and assessment clinics. They will work closely with the medical team to ensure effective patient flow and discharge management is maintained.

Applicants must be highly motivated, have a genuine interest in mental health, and have experience in mental health care.

Job responsibilities

Person Specification Nurse Qualification
  • First Degree level, Independent prescriber.
Responsibility
  • History of holding responsibility and accountability for nursing decisions and empowering nurses to make difficult clinical decisions.
  • Experience of leading on audit and/or research activity related to clinical practice.
Experience
  • Experience of managing a caseload, organising own appointments and managing own diary.
  • Significant experience of CMHT or other community experience.

Salary: £46,148 to £52,809 a year, pro rata.
