Main duties of the job

This post involves working within a multi-disciplinary team supporting children and families in managing diabetes. We provide a quality service meeting the Best Practice Tariff Service Specification and participate in the GIRFT Programme. The team provides support and training from the time of diagnosis whilst an in-patient on the Children's Ward, through to discharge and long term follow-up in the community. We deliver out-patient clinics and offer face to face and video consultations for training and support. Home visits are required as well as visits to schools and nurseries. The team provides structured education sessions to families, schools and ward-based staff. The successful candidate will work as a keyworker for the child and family to coordinate their diabetes care needs and be able to work autonomously.

Minimum Requirements

Candidates should be RSCN / RN child and have experience in paediatric care with particular reference to paediatric diabetes and hold a recognised teaching qualification.

In addition, the candidate should hold a post graduate certificate in diabetes care, or demonstrate a commitment to do so in the future (degree level is desirable).

• Significant post-registration experience, including experience of working in a relevant clinical specialty
• Experience of assessment, development, implementation and evaluation of programmes of care for patients/service-users/staff demonstrating knowledge of specific needs
• Experience of working in an MDT and across organisational and professional boundaries
• Experience of participating in clinical audit
• Experience in policy development
• Experience of developing innovative practice

Qualifications

• Appropriate NMC Registration (RN/RM/RN Child)
• Degree or equivalent qualification (e.g. diploma level specialist training) or experience
• Relevant specialist qualification or training to diploma level or equivalent experience
• Supporting Learning in Practice (SLIP) or equivalent. Or an approved supervisor/assessor preparation accreditation
• Evidence of current CPD

Other requirements

• Hold full UK driving licence with access to own vehicle and holding the appropriate insurance for business use to enable home visits/community work.
