Main duties of the jobThis post involves working within a multi-disciplinary team supporting children and families in managing diabetes. We provide a quality service meeting the Best Practice Tariff Service Specification and participate in the GIRFT Programme. The team provides support and training from the time of diagnosis whilst an in-patient on the Children's Ward, through to discharge and long term follow-up in the community. We deliver out-patient clinics and offer face to face and video consultations for training and support. Home visits are required as well as visits to schools and nurseries. The team provides structured education sessions to families, schools and ward-based staff. The successful candidate will work as a keyworker for the child and family to coordinate their diabetes care needs and be able to work autonomously.
Minimum RequirementsCandidates should be RSCN / RN child and have experience in paediatric care with particular reference to paediatric diabetes and hold a recognised teaching qualification.
In addition, the candidate should hold a post graduate certificate in diabetes care, or demonstrate a commitment to do so in the future (degree level is desirable).

Person Specification Experience- Significant post-registration experience, including experience of working in a relevant clinical specialty
