Job overview

This is an exciting opportunity for a Senior Health Practitioner to join our colleague working in the new pilot service delivering a Primary Care Model of support in partnership with the Alzheimer's Society. Although a prescribing qualification is desirable, it is not essential. The post is for 12 months from recruitment and can be applied for as a Fixed Term Contract or as a Secondment Opportunity. It is ideal for experienced Band 6 practitioners who would like the chance to develop more senior leadership skills and experience.

Current diagnosis rates in Suffolk are below national average rates and yet we know that getting a diagnosis enables people living with dementia to access treatments, plan for the future, access care, and receive essential information and advice.

Working in the allocated GP Practice areas in Suffolk (Burlington, Peninsula and Saxmundham), you will be working with a Dementia Adviser to support assessment and follow-up of those presenting with early symptoms of dementia. You will have the support of the Suffolk Memory Assessment Services for formulating diagnoses, exploring treatment decisions and for Management Supervision.

Main duties of the job

As part of this role, you will:

  • Complete holistic assessments for service users identified by the Dementia Adviser as presenting with cognitive difficulties.
  • Attend the Memory Service MDT to discuss assessments and to formulate diagnoses.
  • Deliver assessment outcomes to service users and discuss next steps.
  • Provide information about healthy lifestyle choices, possible treatment options and signpost back to the Dementia Advisor for ongoing support.
  • Review Service Users for progression of cognitive symptoms and follow up response to treatments, where applicable.
  • Work in partnership with the Dementia Advisor and Social Prescriber affiliated with the GP practices.
  • Attend the Practice MDT meetings.
  • Contribute to providing education to Primary Care colleagues regarding signs and symptoms of cognitive impairment.
  • Provide training sessions about maintaining Brain Health within Primary Care - both for professionals and for those living with health conditions that increase the risk of developing dementia.
  • Contribute to the evaluation of the Pilot.

Training and support will be provided by the Suffolk Memory Assessment Service, and there will be opportunities to shadow practitioners working in the service to learn the assessment process, including the application of the cognitive test.

As this is a Pilot, there is scope to develop the role according to local need, service user response and opportunities for improvement.

Support and guidance will be available from the Memory Assessment Services and Alzheimer's Society colleagues, but innovation is encouraged. New ways of working to address the unprecedented need for support from Dementia services are being explored and you can play a pivotal role in this part of the diagnostic pathway.

Person specification Qualifications
  • RMN or equivalent professional qualification
  • Non-Medical Independent Prescriber.
Experience
  • Extensive experience of practice within clinical area with proven track record in delivering high quality care.
  • Teaching or training experience or qualification.
  • Experience of implementing evidence based practice and or service improvement initiative.
Knowledge
  • Able to demonstrate sound clinical expertise and knowledge of mental health disorders and their treatments.
