Are you a passionate and experienced Clinical Prescribing Pharmacist looking for an opportunity to make a real impact in primary care? We are seeking a dedicated professional to join our dynamic multi-disciplinary team, focusing on medicines management and patient-centred care.
About the Role:This role is designed to enhance patient care through expert clinical practice, prescribing, and leadership in medicines management. You will play a key part in optimising medication use, managing long-term conditions, supporting polypharmacy reviews, and ensuring safe, effective prescribing practices.
Key Responsibilities:- Prescribing & Medicines Management - Lead safe and effective medication optimisation, including structured medication reviews.
- Collaborative Working - Work closely with GPs, nurses, and other healthcare professionals to deliver integrated patient care.
- Patient-Centered Care - Support patients in managing their conditions and improving health outcomes.
- Education & Training - Provide guidance to the wider clinical team on best prescribing practices.
- Quality Improvement - Drive initiatives to enhance medication safety and effectiveness.
- Qualified Prescribing Pharmacist (or other registered health professional) with primary care experience.
- Strong background in medicines optimisation and long-term condition management.
- Ability to work independently and collaboratively in a fast-paced clinical environment.
- Advanced physical assessment skills are desirable but not essential.
- Conduct comprehensive patient assessments, including history-taking and physical examinations.
- Develop and evaluate personalised care plans, with a focus on medicines management.
- Prescribe and optimise medications in line with best clinical practices.
- Provide expert guidance on safe and effective medication use to patients and colleagues.
- Lead initiatives to enhance medication safety and reduce errors.
- Develop and implement medicines management policies.
- Conduct clinical audits and drive quality improvement projects.
- Collaborate with pharmacy, nursing, and medical teams to optimise prescribing.
- Educate and mentor healthcare staff, fostering continuous learning.
- Contribute to the development of clinical guidelines and best practices.
- Represent the organisation at relevant forums on medicines management.
