Community Liver Health Checks (CLHC) Patient Pathway Coordinator

An exciting opportunity has arisen for a Patient Pathway Coordinator for the Community Liver Health Checks project in North East London (NEL) with the Hepatitis C Operational Delivery networks as they embark on a 12 month pilot project.

The role is hosted by Barts Health (NEL ODN) and you will be based within a supportive and friendly team of viral hepatology colleagues as well as a wider MDT dedicated to providing specialist care and support to HCV patients.

The ideal candidate requires a positive "can do" attitude and excellent communication. Previous experience in a similar NHS patient pathway coordinator role would be advantageous, plus a high level of IT ability and excellent analytical skills will also be required.

This is a 9 month fixed term contract but we are also happy to provide a 9 month secondment opportunity. The post holder will support the smooth running of the patient journey of those that have been identified via the Community Liver Health Checks (CLHC) project as requiring HCC / liver surveillance, as well as serving as a point of contact for all patients who engage with the project.

Main duties of the job

The post holder will support the smooth running of the patient journey of those that have been identified via the Community Liver Health Checks (CLHC) project as requiring HCC / liver surveillance. A key aspect of the role may involve daily and direct (e.g. face to face and telephone) communication with patients, relatives, carers, and other health and social care professionals. The information and nature of the communication required is sensitive due to the nature of cancer surveillance communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.

The role will include providing streamlined administration processes around the patient and their individual needs. The role will involve ensuring that clinic appointments are made and communicated to the patient. GP letters are sent to both the GP and patient. Screening tests will be booked by appropriate people. Part of this role will be to ensure that tests have been booked and that patients, their carer (peer support) or hostel provider is aware of the appointment and tests time. The post holder will also maintain a clinical surveillance database, and provide data to the data manager as required.

The role requires use of judgment in responding to the needs of individuals, providing appropriate advice and escalation to the registered practitioner where appropriate.

Person Specification Qualifications
  • Comprehensive secondary education with a minimum of 5 GCSE's
  • Educated to diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent
  • Evidence of some formal further training in short courses, experience of a range of admin practices and significant experience of hospital administration
  • Evidence of further education
Experience
  • Significant proven experience of delivering to high standards in a fast-paced high volume customer care environment
  • Ability to deal with a range of issues independently and efficiently
  • Experience of an administrative role with the NHS
  • Experience of supervising staff
  • Experience of working across organisational boundaries
  • Experience of working with IT Healthcare Systems, e.g. Cerner, Somerset, EPR
