This post offers a great opportunity for a RMN/RGN or dual qualified nurse to utilise their specialist skills, working in a dedicated, friendly and supportive team. We are looking for a compassionate, motivated and caring individual who has experience in working with people with functional and organic mental illness with or without frailty.
The successful candidate will work as a member of the South East Essex Older Adult's Community Mental Health Teams as a physical health lead, combining pre-existing mental and physical health skills, in order to promote the health and wellbeing of our patients.
Main Duties of the Job- The post holder carries responsibility for the assessment of care needs and the development, implementation and evaluation of programmes of care for patients with Serious Mental Illness (SMI).
- To work in partnership with health care professionals and other agencies to reduce the impact of poor physical health on the client group, improving outcomes.
- To support the Community Mental Health Nurses in the physical health care of individuals with Serious Mental Illness (SMI).
- To provide physical health assessments as the service requires.
- To support other professionals in the service in undertaking physical health assessments.
- To work collaboratively with primary health, developing and maintaining links with other professionals.
- To have accountability and responsibility for implementation of physical health pathways, to support the SMI physical health agenda.
- To work across the service offering physical health support and advice to team members as required.
- To support team members in providing care to complex cases where physical health needs are identified.
- Registered Mental Health Nurse /Registered General Nurse or Dual Qualified General / Mental Health Nurse.
- Evidence of personal and professional development in contemporary mental health practice and dementia care.
- Good working knowledge of physical health management strategies and interventions.
- High level of clinical knowledge and skill including knowledge of recent national, regional and local health strategies.
- Experience of working within mental health community services.
- Knowledge of the physical health impact from diagnosis of and treatment for serious mental illnesses (SMI).
