Job Description
Community Sister/Charge Nurse/Caseload Holder Livewell Southwest appreciates that work life balance is vitally important for our employees' wellbeing and therefore we have hours available from 22.5 - 37.5 hours per week on permanent contract. Our core hours are 0800 - 17:30 and are open to discuss any working pattern within these hours, to enable you to get the balance of home and work life that suits you.
The role requires positivity and excellent communication, leadership, and caseload management abilities. You will need to demonstrate integrity and initiative, sound clinical knowledge with evidenced based practice and the ability to manage a team, approaching your work in a coordinated way.
We work integrated with primary care, ASC, therapists, urgent & intermediate care services alongside our specialist services informing our ageing well MDTs, with a focus on TEP and enhanced care planning. In patients' homes and part of the Care Home Service.
If you have an interest in community nursing, preventing hospital admissions, and caring for people with complex long-term disease, through to the last stages of their life, this role is for you.
This role may not be eligible for sponsorship under the Skilled Worker route.

As a Community Sister/Charge Nurse, your role is vital in terms of coordinating people's care & delegating to your team.
You will be required to be an autonomous practitioner, provide holistic patient-centred care. To champion for patients, whilst demonstrating highly motivated leadership skills with the ability to influence/inspire others. The candidate must have the knowledge and confidence to have delicate conversations around advanced care planning in anticipation of end of life care including treatment escalation and have the skills required to make clinical decisions around resuscitation status based on the person's clinical presentation.
You will be required to undertake a robust induction, allocated a supervisor to support your learning to develop in your role.
We offer training in line with the required competencies for you to build on your past knowledge/experience/skills. You will be expected to undertake continued learning with accessibility to accredited courses.
You will be required to be competent to diagnose/investigate. Have sound knowledge/judgement providing evidence-based wound management/pressure ulcer prevention, continence management, palliative & end of life care.
Have a full driving licence and the ability to travel across Plymouth.
All LWS staff are expected to be able/willing to work across a 7 day service.

Be able to respond to urgent referrals for patients, carers and the multi-disciplinary team.
To triage and accept appropriate referrals directly from other disciplines, patients, carers and members of the public and/or refer to other agencies where appropriate.
Be responsible for assessing, diagnosing, planning, organising & reviewing complex packages of care including case conferences.
The candidate must have the knowledge and confidence to have delicate conversations around advance care planning in anticipation of end of life care including treatment escalation and have the skills required to make clinical decisions around resuscitation status based on the person's clinical presentation.
Ensure privacy, dignity and human rights are respected for all patients and carers.
Undertake comprehensive risk assessments of all situations associated with the care of patients in order to ensure nurses and the carers' safety.
Assessments might include the provision of equipment ensuring it is used safely.
Be responsible for overseeing, monitoring of equipment and ensuring maintenance & repair.
To report and record all incidents and near misses relating to health, safety, security, fire, physical violence, aggression and verbal abuse.
Maintain accurate records, which are confidential, up to date, legible and that all care given has to be documented. These records may be paper or IT based system and must be maintained as specified in the Trust Policies, NMC guidelines and Government directives. The post holder will be expected to oversee the standard of patients' records by audit or peer review and have a regular open discussion with team members.
The post holder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager. If no action or explanation is received, then it is the individual's responsibility to escalate to the Director of Operations or Chief Executive Office.
To monitor & maintain the health, safety and security of self and others.
Be responsible for organising own time management on a daily basis in line with caseload demands balancing patient needs with the non-clinical aspect of the role.
Work within challenging and unpredictable environments e.g. patients' homes, which may have constraints that require adaptability.
Act as a clinical advisor on health care issues within their area of expertise.
Undertake health promotion whenever possible to all age groups and also contribute to planned health promotion activities.
Nurse Prescriber and takes responsibility for prescribing in line with Non-medical Prescribing policy and within the Devon Formulary.
Identify community health needs and participate in new projects, research and audit.
Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.
Be involved with the development, implementation and monitoring of standards.
Develop links with other professional and voluntary bodies to ensure maximum effectiveness of the service and patient care.
Work alongside the Community Matron for Long Term conditions if appropriate in monitoring complex health needs of the patients and reduce hospital admissions.
Ensure that work which is often unpredictable is prioritised and suitably delegated to other members of the multi-disciplinary team.
Implement, participate and facilitate Clinical Supervision as per organizational Policy.
Participate in an annual appraisal of their work in line with the Knowledge and Skills Framework (KSF) where the job description will be reviewed and objectives set. In line with the annual development plan, the post holder will be expected to undertake any training or development required to fulfil their role.
Be conversant and adhere to the vulnerable adult and child protection procedures & Organizational policies.
Support the District Nursing Manager regarding equity of workload throughout the larger Team & deputise in his/her absence.
