An exciting opportunity has arisen to join our vibrant Adult Community Nursing Service. Homerton Healthcare NHS Foundation Trust is an integrated community and acute trust providing high quality service to residents of City and Hackney borough.
We are seeking enthusiastic Band 5s to fill a full-time (37.5) permanent post. The Adult Community Nursing Service is divided into 8 neighbourhood teams. The successful candidate will be asked to work within one of the neighbourhood teams.
Main duties of the job- The post holder will practice within a multidisciplinary team and provide care to patients within their own home and community settings.
- Assessing, planning, implementing, and evaluating their care needs under the supervision of Clinical Nurse Specialists, Practitioners, and District Nurses.
- The successful candidate will be responsible for delivering a high standard of holistic nursing care whilst promoting and maintaining health education and independence.
- The post holder will take responsibility for their own actions when delivering clinical care, maintaining competency and skill in line with role requirements.

- Community experience working (this could include a period working on student placement in the community)
- Experience of multi-agency working
- Able to work unsupervised in the community setting
