Job overviewAre you what we are looking for? If you consider yourself to be the next generation of leaders in the NHS, an exciting opportunity has arisen for a Band 6 Nurse in the Outer North West Division for a community nurse to work closely within the planned care of Nursing and Therapy.
This is a fixed post position with the possibility of becoming permanent in Brent at the end of this contract.
This is a great opportunity to develop your understanding of quality drivers and work closely with clinical teams to support the delivery of high-quality care and positive patient and staff experience.
We are looking for a dynamic and motivated individual with nursing experience who can provide local clinical leadership for the multi-disciplinary team working across the Division.
Main duties of the jobThe post holder will work in collaboration with the designated Deputy Team Leader in managing the care of patients with long-term conditions, incorporating monitoring progress and promoting the preventative aspects of nursing practice.
The successful candidate will have the opportunity to further develop quality expertise and lead key projects by developing strong relationships across the Division.
You will work collaboratively with the wider multi-disciplinary team, other health and social care practitioners, and the nursing staff to provide the best care and quality outcomes for patients and service users, in keeping with national guidelines.
Job Purpose- To provide skilled nursing care working as part of Central London Community nursing service to patients in various community settings between the hours of 0830 and 2100, including weekends.
- To actively monitor, maintain and develop the service provided.
- To take responsibility for managing the caseload and the team in the absence of the DN team lead.
- To deliver a case-managed approach for patients with long-term care needs.

Person specification Education/Qualification- RGN/RN with current registration
- Evidence of recent and continuing professional education and training
- Practice supervisor/assessor training
- Formal training in staff and team management
- Experience of negotiating and liaising within a multi-disciplinary team
- Experience of working with people with complex health needs
- Experience of supervising junior staff and coordinating a team regularly
- Experience of teaching students and other staff members
- Experience of implementing evidence-based care in practice
- Experience of undertaking comprehensive health needs assessments and writing care plans
- Experience of managing staff
- Post-registration community nursing experience
- Principles of care for patients with long-term conditions, based on local/national guidelines and standards
- Understanding of NHS and integrated care boards structures
- The principles of clinical governance and application to practice
- Able to identify own and others' learning and development needs and actively seek ways to meet those needs
- Able to carry out the duties of the post with or without adaptations
- Able to provide evidence of demonstrating the Trust's Values and Behaviour
- Able to demonstrate excellence in Customer Service
