Diabetes Specialist Nurse

We have an exciting opportunity to join our team as a Diabetes Specialist Nurse, supporting patients with diabetes under the RUH. The post holder will have two main roles: supporting young adults with diabetes and supporting women with diabetes planning pregnancy. The post holder will support people with type 1, type 2 and other forms of diabetes. They will be responsible for the provision of pump and specialist technology treatment and support, including the use of hybrid closed loops. They will provide expert resources for persons with diabetes, medical, nursing and allied healthcare professional staff, facilitating communication between primary, secondary care, the integrated team and Integrated Care Board (ICB). They will be responsible for a caseload of patients providing advanced clinical support, advice, treatment and education for people, avoiding hospital admission, by prompt intervention and treatment.

Main duties of the job

The post holder will be responsible for a caseload combining high-level assessment, advanced diabetes specialist knowledge, initiating diabetes treatments, and appropriately leading the co-ordination of care referring to other health professionals within the multidisciplinary team, integrated team, mental and social care.

1. To initiate insulin and injectable therapies and provide ongoing support within current prescribing guidelines.
2. To initiate continuous glucose monitors and hybrid closed loop therapy and provide ongoing support for patients using these technologies.
3. To support young people with diabetes, particularly high-risk patients who struggle to manage diabetes and those who have difficult engaging with diabetes services.
4. To work closely with the diabetes and antenatal team, to support women during the pre-conception period.
5. Working as an autonomous practitioner in partnership with other care professionals, using advanced skills, and expert knowledge to assess, monitor, and review the complex physical, social and psychological needs of the patients with diabetes. Develop care plans and instigate therapeutic treatments based on comprehensive patient focused assessments and best available evidence in order to improve health outcomes.

Person Specification Qualifications and Training

• Registered Nurse with NMC registration
• Independent nurse prescribing (2) or willing to take (1)
• Evidence of training at degree level and ongoing continuous professional development
• Participated in conferences, training associated with diabetes management, pump therapy and technologies
• Accredited teaching qualification

Knowledge and Experience

• Demonstrable experience working with persons with diabetes either within the community or acute setting
• Up to date knowledge of nursing theory and best practice, particularly within diabetes nursing and an understanding of its application in clinical practice
• Experience managing and supporting persons with insulin pump therapy

Specific Skills

• Advanced skills in the assessment and treatment of patients with diabetes
