Job overviewWe have an exciting opportunity for a Band 6 Health Visitor to join one of our forward-thinking and innovative integrated 0-5 Intensive Support Teams in Westminster.
You will be an integral part of the Local Authority Targeted Early Help Team and be reporting to and working alongside the Intensive Team Lead to deliver the 0-5 Intensive Support program.
You will have a small caseload of up to 25 families with multiple and complex needs under the supervision of the Intensive Team Lead and Early Help Practice Manager, with the aim of supporting families to facilitate change thus improving outcomes for their children. The work will be guided by Systemic Practice and the Early Help Framework and operating model.
Contributing to the delivery of Working Together to Safeguard Children (2018), including improving health and life chances for children and families and safeguarding those at risk.
For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C's apply)
Main duties of the jobOur integrated team works passionately to deliver our progressive service and continue to seek and develop innovative ways of working with Westminster families to enable children to have their best start in life. Children, young people, and families are our focus and passion, and we are inviting passionate Health Visitors, with a desire to work more intensively with families, to join our dynamic diverse team. We champion the role of community health professionals to make sure our families get the best care closer to home. The current climate means this is more important than ever; ensuring that all families access face-to-face services and only have to tell their story once.
The intensive 0 - 5 programme is a new, early intervention home visiting programme delivered over the course of a year. The underlying aim of the intervention is to enhance parent-child relationships as a means to improving child development and school readiness thereby preventing escalation to statutory intervention.
You will assist the Team Lead to deliver the 0-5 intensive support programme and will have the opportunity to develop high levels of professional skills and practice as well as the ability to work as part of a team.

Person specification Education/Qualification- Registered Nurse/Midwife with current UK NMC registration
- Registered Health Visitor (SCPHN)
- Evidence of continuing post-registration education and training
- Practice assessor/supervisor training or working towards
- Able to carry out the duties of the post
- Able to travel round the locality (car/bike/public transport/foot)
- Able to provide evidence of where you have demonstrated the Trust's Values and Behaviours
- Able to demonstrate excellence in Customer Service
- Able to meet the required IT Skills for the post
- Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets
- Experience of working collaboratively as part of a multidisciplinary team and inter-agency working
- Experience of organising and managing own workload and that of others
- Experience of assessing, planning and implementing programmes of care and writing care plans
- Experience in providing practice-based learning support to others
- Experience of evidence-based care
- Experience of supervising staff/students
- Knowledge of current health and social care agenda relevant to area of work and professional practice
- Knowledge of current health legislation (including Change for Children agenda, Children's Plan etc.)
- Current knowledge of child protection issues
- Good communication skills, both written and verbal
- Able to meet the required IT Skills for the post
