About the Role:

We're looking for Locum Clinical Pharmacists to join our small but exceptional Care Team. This role will report into a Clinical Team Lead and is at a rate of £40 per hour, with a minimum commitment of 3 days and 24 hours at least. This role is ideal for those looking for flexibility and remote work!

What you'll be doing :

On your best days, you will play a key role in our Care team, providing specialist pharmaceutical support to patients that Suvera is supporting via our NHS Partners. Your active role will offer you the unique chance to be part of shaping a new future in patient care, ensuring our patients safety and dignity is maintained at all times and ensuring they are supported with high quality chronic disease management, repeat prescription management and structured medication reviews.

As a Locum Clinical Pharmacist, you will:

  • Carry out reviews for patients with long term conditions within your clinical competencies and provide specialist support to patients that Suvera is supporting via our NHS Partners. Current conditions we manage are listed below in our 'must haves'.
  • Work across multiple partner organisations, the role will have a predominantly patient-facing element, including chronic disease management, repeat prescription management and structured medication reviews.

Clinical Responsibilities

  • Collecting and recording patients' health data in a holistic manner.
  • Interpretation, Risk stratification, and Management of this data in line with standard operating procedures and NICE guidelines.
  • Able to provide evidence based lifestyle advice.
  • Prescribing / altering medications for patients with LTCs.

Non Clinical Responsibilities

  • Maintaining at all times a regard for patient safety and dignity.
  • Building and maintaining trust with patients.
  • Maintaining contemporaneous clinical notes within Suvera's and GP Partners systems.
  • Being proactively involved in Quality Outcomes Framework (QOF).
  • Undertaking other administrative duties where reasonably required.
We're looking for someone who:

Should have:

  • A minimum of 12 months experience as a clinical pharmacist within an NHS Primary Care setting.
  • A current member of a medical professional defense organisation.
  • An independent prescriber and have completed a GPhC-accredited course/or equivalent.
  • Currently registered with the GPC and act at all times within the boundaries of its code of Professional Conduct.
  • Excellent communication skills, both written and spoken.
  • Confidence in communicating with patients via their preferred communication channel which could include email, messaging, telephone, or video calls.
  • IT literacy, and able to work with a variety of software systems, including but not limited to Emis Web, SystmOne, Microsoft Office.
  • Confidence in managing the following conditions: Hypertension, Type 2 Diabetes, Asthma, Lipids, Structured Medication Reviews, Frailty, CKD.
