Mental Health Nurse Practitioner- Hope Service

The Hope service is county-wide and during the hours of operation, 9am-5pm, Monday to Friday operates from Epsom or Guildford site. Assessment, support and treatment are offered to CYP known to CYPS community teams and/or Surrey Families' Directorate. There is a structured Day Programme as well as the Outreach provision working in community settings. The Service also provides transitional support for those CYP in hospital (acute or T4 adolescent provisions) and who are returning to their local community. The Service considers referrals to avoid hospital admission or facilitates early discharge from hospital. Referrals may also be received from the CYPS Community Teams SCC Families Directorate.
Main duties of the jobMain duties of the role include:
- Assessment and interventions related to Mental Health Crisis for young people aged 10-18
- Care Coordination
- Mentorship and training for colleagues
- Supporting families through systemic practice
- Leading on the planning and risk profiling for care delivery in the community
- Working with partner agencies and inpatient care facilities to promote safe support for young people in the community
- Support and maintain essential clinical governance functions of the service

Job responsibilitiesThe job description for this role is attached. The band 7 RMN's in this team are part of a multidisciplinary service including Doctors, Nurses, Social Workers, Activity Workers, Therapists and Managers.
Person Specification Qualifications- RMN with experience of working with Children and Young People
- Registered Nurse (Child)/ RSCN with Children and Young People's Service experience with Mentorship preparation, relevant teaching qualification or relevant experience
- Assessment in Practice or Mentor preparation course
- Experience of working within Crisis Pathways/provisions for young people
- Experience of undertaking specialist assessment and treatment of Children and Young People
- Experience of working with Children and young people either in community or T4 settings
- Experience of family therapy or therapeutic intervention skills
- Level 3 Safeguarding Children and Adults training
- Experience of working with disordered eating
