Job Description
About the Role:

We're looking for a Clinical Pharmacist to join our small but exceptional care team for a 3-6 months fixed term contract with the potential to extend or go permanent. This role will report into a Clinical Team Lead and offers a salary of up to £50k DOE.

What you'll be doing :

As a Clinical Pharmacist, you will:

  • Carry out reviews for patients with long term conditions within your clinical competencies and provide specialist support to patients that Suvera is supporting via our NHS Partners.
  • Work across multiple partner organisations, with a predominantly patient-facing element, including chronic disease management, repeat prescription management and structured medication reviews.
  • Be proactively involved in quality improvement and audit as well as managing some aspects of the Quality and Outcomes Framework.
  • Be supported to help develop the role as the clinical team grows.

Clinical Responsibilities

  • Collect and record patients' health data in a holistic manner.
  • Interpret, risk stratify, and manage this data in line with standard operating procedures and NICE guidelines.
  • Provide evidence-based lifestyle advice.
  • Prescribe/alter medications for patients with LTCs.
  • Provide clinical leadership to future members of the clinical team.

Non Clinical Responsibilities

  • Maintain at all times a regard for patient safety and dignity.
  • Build and maintain trust with patients.
  • Maintain contemporaneous clinical notes within Suvera's and GP Partners systems.
  • Be proactively involved in Quality Outcomes Framework (QOF).
  • Liaise with and assist members of the operations/product/technology teams.
  • Support and participate in shared learning within Suvera and our GP Partners.
  • Take a proactive role in audits and statistics relating to patient activity.
  • Assist in the development and improvement of Suvera's systems and services.
  • Undertake other administrative duties where reasonably required.
We're looking for someone who:

Should have:

  • A minimum of 12 months experience as a clinical pharmacist within an NHS Primary Care setting.
  • A current membership of a medical professional defense organisation.
  • Independent prescriber status and completion of a GPhC-accredited course/or equivalent.
  • Registration with the GPC, acting at all times within the boundaries of its code of Professional Conduct.
  • Excellent communication skills, both written and spoken.
  • Confidence in communicating with patients via their preferred communication channel.
  • IT literacy and ability to work with various software systems, including but not limited to Emis Web, SystmOne, Microsoft Office.
  • Confidence in managing conditions such as Hypertension, Type 2 Diabetes, Asthma, Lipids, Structured Medication Reviews, Frailty, CKD.
