The Crisis Resolution and Home Treatment Team for older adults is a well-established, friendly, and supportive multidisciplinary team based in Fulbourn, Cambridge. The team offers intensive support and treatment at home and in care homes for older adults with mental health challenges, including behavioral and psychological symptoms of dementia. Our services include crisis intervention, intensive treatment, risk assessment and management, dementia intensive support, and facilitating early discharge from mental health hospitals.
We are a passionate, fun-loving team with extensive clinical experience. We seek a team member who shares our dedication to working with older adults, is motivated to develop their skills, can work under pressure, and has experience in clinical assessments and risk management. A sense of humor and teamwork are essential, as well as the ability to contribute to team meetings and potentially lead within the team.
Main Duties- Provide a community-based mental health service, including psychosocial assessments, seven days a week.
- Work with service users and carers to deliver treatment in the least restrictive environment.
- Act as a care coordinator, leading assessments, care planning, and reviews.
- Assess risk of hospital admission and support community-based care or facilitate hospital discharge.
- Conduct clinical risk and mental health assessments and contingency planning.
- Promote effective teamwork and serve as a role model.
- Deliver crisis and home treatment services, including dementia support.

- Deliver evidence-based clinical interventions and make autonomous decisions.
- Develop and implement brief care and treatment interventions.
- Ensure timely and effective delivery of care episodes.
- Empower patients and carers in decision-making and care ownership.
- Mental Health Nurse, Social Worker, or Occupational Therapist qualification.
- Approved Mental Health Practitioner qualification or equivalent experience.
- Relevant mental health treatment experience.
- Experience working with complex mental health needs.
- Teamwork and service user involvement experience.
- Teaching or supervision experience preferred.
- Assessing and working with distressed individuals.
- Clinical risk assessment and contingency planning.
- Effective communication of complex information.
- Crisis intervention strategies.
- Ability to develop new initiatives.
- Knowledge of relevant legislation and models of care.
- Proficiency in SystmOne and relevant clinical courses.
- Ability to travel across a large geographical area promptly.
