Main duties of the jobTo work as a senior staff nurse and deliver nursing support and care to patients being managed on the virtual ward pathways.
To provide direct clinical care to patients on the Virtual Ward as part of the multi-disciplinary team with supervision and support from the Advanced Clinical Practitioners and medical team.
To be based predominantly in community care settings while working collaboratively across secondary care and community care to support the development and delivery of Virtual Wards across LSCFT and LTH sites.
To assume delegated responsibility for appropriate deployment and supervision of staff for the effective management of a defined caseload.
To provide direct nursing care in the assessment, planning, implementation and evaluation of high quality evidence based nursing care to patients in their own homes and other community settings for a defined cohort of patients to ensure that the care needs are met.
To work as part of a hospital at home model within Care Connexions to support admission avoidance and support patient triage for those in a health care crisis.
To work to a defined shift pattern within a service responsible for the provision of care 8am-8pm 365 days per year taking responsibility as delegated by team leader/manager to lead a defined shift at team level.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.

Person specification Qualifications- RN- Adult- NMC registration
- Educated to degree level
- Evidence of CPD
- Understanding of Clinical Governance / Standards for Better Health agenda
- Good understanding of virtual ward / 2 hr UCR services
- Knowledge of Primary and Community Care Services
- Working at B5 nurse
- Working as part of a team
- Ability to work without direct supervision
- Risk Management knowledge and experience
