Job Description
Advanced Clinical Practitioner - Vascular Nottingham University Hospitals NHS Trusts

This is an exciting opportunity within the Vascular directorate to work as an Advanced Clinical Practitioner (ACP).

The Acute GI ACP will be responsible for integrating ED, SDEC and virtual ward work streams into the broader Acute GI service. The ACP will work closely with Vascular nurse practitioners and the Vascular clinical lead to develop the service and ensure a robust pathway of care for our patients.

Main duties of the job

The role includes:

  1. Clerking patients in ED and SDEC, facilitating access to diagnostic imaging/procedures, ensuring ward-based procedures are completed as needed (e.g. paracentesis) and completing discharge summaries for SDEC patients.
  2. Preparing entries for patients in the Vascular Virtual Ward, attending the consultant-led Virtual Ward round three times per week. Co-ordinating care for patients on the virtual ward by linking closely with the community and hospital VW team.
  3. Capturing data pertaining to SDEC and Virtual ward in conjunction with coding team/analysts to demonstrate the value of these services to the wider department and to the Trust.
  4. Development of specific outpatient work streams for trainee and qualified ACPs with consultant oversight.
  5. Innovation and expansion of SDEC/Virtual Ward pathways in close conjunction with the Vascular clinical lead.
  6. When not in ED/SDEC/Virtual ward, responsibilities will include specialist in-reach for patient referred to Vascular across the Trust.
  7. Attendance at thrice weekly virtual ward meeting.
  8. Daily AM presence in ED and SDEC.
  9. Development of ACP Acute GI role to incorporate semi-elective work e.g. Ward Discharge clinic.

Person Specification Qualifications
  • Msc in Advanced Clinical Practice
  • Registered Nurse or Allied Health Professional registered with a professional body
  • Non Medical Prescriber registered with NMC or other professional body
  • ALS Provider
  • Evidence of post registration training and/or qualifications
  • Evidence of teaching, supervision and assessment skills
  • ALS Instructor
Experience
  • Completed a portfolio of supervised practice in Advanced Clinical Practice
  • Evidence of continued professional development following completion of Msc in Advanced Clinical Practice
  • Management and leadership experience
  • Able to present and receive complex information to multidisciplinary team
  • Able to work independently and within a team
  • Ability to communicate effectively with wide variety of patients
  • Assertiveness and conflict resolution skills
  • Ability to accept constructive feedback
  • Evidence of multidisciplinary teaching
Analytical and Judgement Skills
