You will work as part of the multi-disciplinary team here at the surgery, and take on additional training should it be needed.

Main duties of the jobThe post holder will demonstrate critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Job responsibilities- You will provide a holistic approach to healthcare, drawing from both the nursing and medical model of health and illness where appropriate to screen for the early signs of disease and diagnose acute problems.
- Accept referrals from other disciplines unsupervised.
- Undertake the independent and autonomous assessment of undifferentiated and undiagnosed problems with attention to physical, emotional, social, cultural and mental health issues.
- Use sound judgement in assessing conflicting priorities and needs.
- Formulate a diagnosis utilising medical diagnostic problem-solving skills including history taking, physical examination, invasive and non-invasive investigations which involves contact with potential infectious biological substances on a regular basis.
- Initiate appropriate diagnostic and therapeutic interventions including radiology and laboratory investigations, with attention to safety, cost, invasiveness, acceptability and efficacy.
- Prescribe pharmaceutical interventions within own competency.
- Interpret and evaluate diagnostic test results in liaison with the patient and initiate treatment accordingly.
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
- Contribute to the practices management of patients needing regular review including those with dementia, mental health problems, and learning disabilities.
- Participate in the administrative and professional responsibilities of the practice team.
- Face to Face appointments and telephone triage.
- Qualifications: Registered first level nurse, MSc or equivalent, Relevant nursing/health degree.
- Experience of working in Primary Care.
