The future is exciting. Are you ready to join Primary Care Sheffield (PCS) as one of our Advanced Nurse Practitioners? Our growing collaboration within health and social care in Sheffield provides PCS with a wide range of opportunities which can improve patient pathways and reduce costs.
This post is for 10.5 hours per week.
There is also scope to work at the Anima Hub, for up to full-time hours per week (37.5).
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice.
The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the PSC practices to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required.
In order to work at this level, NMC requirements for advanced practice must be met. To be considered for this exciting role, you must have: a minor illness qualification and a Non-Medical Prescribing qualification (V300).
The work could be as part of a group-wide triage team or within one of our seven practices.
Main duties of the job- Comprehensively assess, review, and evaluate the needs of patients with undifferentiated healthcare needs.
- Undertake clinical assessment and provide the necessary treatment.
- Request diagnostic tests only where clinically indicated in line with organisational, local, and national guidelines and where appropriate.
- Develop a plan of care in conjunction with the patient.
- Utilise effective communication to aid onward referrals to specialists as appropriate.
- Work in an autonomous manner without direct supervision whilst recognising limitations of the practitioner role.
- Carry out enhanced clinical skills as required.
- Keep accurate and immediate records of all interventions fully utilising software systems in place.
- Encourage clients to carry out self-care as appropriate.
