Job summaryThe PCN Advanced Paramedic Practitioner role at Farnham Integrated Care Services (FICS), Farnham Centre for Health, will be a registered Paramedic with post graduate qualifications, including the Independent and Supplementary Prescriber qualification and HCPC registration, acting within their professional boundaries.
Main duties of the jobThe following are the core responsibilities of the PCN Advanced Paramedic Practitioner:
- Alongside the multidisciplinary team, provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilizing skills in history taking, physical examination, problem-solving, and clinical decision-making to establish a diagnosis and management plan.
- Provide assessment, treatment, and diagnosis at point of first contact by attending to patients within our Same Day Service at FICS.
- Make professionally autonomous decisions for which he/she is accountable.
- Refer patients to an alternative care setting or treat and discharge as appropriate.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
- Generate EPS and FP10 prescriptions within the scope of practice, considering local and national guidelines.
- Ensure that professional standards are maintained within the guidance provided by the Department of Health, the Health and Care Professions Council (HCPC), and the College of Paramedics (COP).
- Support the development of excellent relationships across the PCN to enable collaboration for better patient outcomes.
- Support network implementation of agreed service changes and pathways, working closely with other staff members, the wider PCN, and the commissioner to develop, support, and deliver local improvement programmes aligned to national and local priorities.
Farnham Primary Care Network (PCN) is a group of practices working together to focus on local patient care. Our work is a collaboration with Downing Street Group Practice, Farnham Dene Medical Practice, and Farnham Park Health Group. We provide a wider range of services to support our patients.
The main purpose of our PCN is to enhance patient outcomes, improve the quality of care, and increase the efficiency of healthcare delivery. As a PCN, we achieve these goals through improved coordination of care, offering a wide range of services, enhancing access to care, managing population health, promoting preventive care, health education, and prioritizing a patient-centered approach.
Our ultimate aim is to deliver better health outcomes for the local population, as well as a reduction in health inequalities. We work alongside Farnham Integrated care services which houses some of our clinicians, the same day service, and enhanced access.
Job descriptionThis role will work as part of a multi-disciplinary team of AHPs/GPs/HCAs, providing care for the presenting patients within our Urgent Same Day Care Service at FICS. An established integrated service supporting the GP Practices of Farnham.
From initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care, the PCN Advanced Paramedic Practitioner will demonstrate safe clinical decision-making and expert care for patients within the service.
The PCN APP is expected to attend to patients within the defined list of conditions, including assessment, diagnosis, and treatment to deliver quality patient services in conjunction with the daily duty doctor.
The PCN APP will work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery as per policy and procedures. There is an expectation that training across the service is maintained to an expected level with the use of appropriate equipment and facilities as directed by the post holder, who is an autonomous allied healthcare professional and will be part of PCN support to practices while working at FICS.
Person Specification Qualifications Essential- Registered paramedic
- Evidence of continual learning and professional development
- Minimum of two years working as an Advanced Paramedic
- An appreciation of the nature of GPs and an urgent Care Centre. An appreciation of the nature of Primary Care prescribing, concepts of rational prescribing, and strategies for improving prescribing
- Awareness of telephone triage assessment
- Management of common minor injuries/illnesses
- Clinical examination skills
- Knowledge of needs of patients with long-term conditions
- Management of long-term (chronic) conditions
- Clinical knowledge and understanding of the principles of evidence-based healthcare
- Understanding of the mentorship process
- An appreciation of the new NHS landscape including the relationships between FICS, PCNs, and the commissioners
- Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face to face
- Willingness to work flexibly to deliver safe and effective healthcare
- Ability to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct
- Ability to use own initiative, discretion, and sensitivity
- Able to get along with people from all backgrounds and communities, respecting lifestyles and diversity
- Ability to identify risk and assess/manage risk when working with individuals

- Knowledge of IT systems, including ability to use word processing skills, emails, and the internet to create simple plans and reports
- Clear, polite telephone manner
- Good clinical system IT knowledge of EMIS
- Ability to promote best practice regarding all pharmaceutical matters
- Effective time management (planning and organising)
- Demonstrate personal accountability, emotional resilience, and work well under pressure
- Evidence of continuing professional development
