JOB OVERVIEW
There is an exciting opportunity to join the North Older Person's Community Mental Health Team. Our base is located close to Norwich City Centre at the Julian Hospital. Our Team is rated good by the CQC and we have recently received a Research Impact Award. We have a long history of excellent staff retention and a culture of coaching and supporting our staff to deliver high quality care.
The trust offers a relocation bonus of up to £5000, depending on distance of relocation.
Main duties of the jobThe successful candidate will work with service users with Dementia or suspected Dementia, functional mental health conditions, such as those on the SMI register, depression, and anxiety. Responsibilities include:
- Planning, reviewing, and delivering care and treatment in the community in conjunction with service users.
- Providing duty cover for the team on a rostered basis.
- Working flexibly on a busy caseload of around 20-30 service users.
- Supervising peers and students if requested.
- Identifying personal learning and training needs.
Working hours are currently 9am-5pm Monday to Friday. The successful candidate will have the opportunity to work flexibly as per the NSFT flexible working policy.

As a Community Mental Health Practitioner, you will be responsible for:
- Holding a caseload of patients with organic and functional presentations under CPA and NCPA
- Having sound knowledge of the Mental Health Act, Mental Capacity Act, NICE guidelines etc.
- Taking new referrals from GP's, Wards and other mental health teams/services.
- Performing initial assessments and reviews.
- Assessing and managing risk.
- Using Dialog+ and Recovery Care Planning with service users and carers.
- Managing your own diary and planning your day.
- Attending MDT meetings and ward review meetings.
- Being part of the duty system, usually once per month.
- Communicating effectively through clinical notes, documents on Lorenzo, and letters to GPs and other involved parties.
- Driving is essential for this role.
- Experience of working within a multidisciplinary team
- Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
- Experience in delivering group-based interventions
- Having knowledge of working with people with dementia
- Risk assessment knowledge and skills
- Prevention and Management of Suicide training
- Able to time manage and reflect on clinical practice
- Knowledge of psychosocial interventions
- Developing knowledge of local wider systems and networks
- Ability to travel independently and have access to a car
- Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent
- Good to great management course, or equivalent
