Job Description

Full-time - 37.5 hours per week - job share/part time may be considered

This is an exciting opportunity to work as part of the multi-disciplinary Diabetes Service with Bromley Healthcare, based at Beckenham Beacon and sites across Bromley.

It is an opportunity for someone with an enthusiasm for diabetes and a wish to extend their expertise in this field. The post will include responsibility for a mixed caseload including people with Type 1, Type 2 and gestational diabetes. You will need to be confident in delivering education to groups and ideally have experience of structured education programmes e.g. DESMOND/DAFNE.

You will be involved in the development of diabetes within the Bromley Healthcare Diabetes Service. You will need to show empathy and sensitivity to the needs of the patients, have good communication skills, together with a friendly, polite and professional attitude.

You will need excellent clinical, organisational, IT and communication skills.

Main duties of the job
  • Provide comprehensive, evidence-based diabetes care for people with diabetes and healthcare professionals within Bromley.
  • Work as a highly-skilled independent practitioner implementing and recommending treatment based on expert clinical judgement.
  • Provide comprehensive evidence-based highly specialist clinical care, training, and consultancy to both people with diabetes and other professionals. Providing advice, support, and education to GP Practices, and providing care in a range of settings including the diabetes centre, patients' homes, and nursing homes.
  • Contribute to the development of diabetes services across provider boundaries.
Job responsibilities

To provide an expert diabetes nursing service, giving support, advice, and information about diabetes to patients and their significant others in the diabetes center, hospital, or home plus community and primary care.

Act as an expert resource, providing clinical advice and support to others involved in providing care for people with diabetes, in both primary, secondary care, and community.

Work as an independent practitioner and demonstrate specialist skills and knowledge through expert nursing.

Practice advanced clinical skills and maintain a high professional standard of care and expertise, acting as a role model for other healthcare professionals.

In conjunction with Diabetes Service Lead and Consultant Clinical Lead, assess referrals to the diabetes center, prioritise and decide when patients need to be seen and by whom (i.e. Consultants and Nurses), and refer patients to other services (Podiatry, Dietetics etc.).

Assess, advise, and take appropriate action for walk-in patients, emergency and routine phone calls from GPs, practice nurses, district nurses, hospital-based staff and other healthcare professionals.

Be responsible and accountable for managing a varied caseload of patients, including patients with multiple medical conditions and complex needs, prioritising the workload.

Organise and undertake comprehensive assessments and interpret clinical data including blood biochemistry for the treatment for people with both Type 1 and Type 2 diabetes.

Undertake home visits for the assessment and review of housebound patients with diabetes.

Provide support and education to GPs and practice nurses and make recommendations relating to the adjustment of medication and treatment and the need for diagnostic tests.

Work with a range of agencies and healthcare professionals including District Nursing, learning disabilities and nursing homes and effectively communicate information to ensure continuity of care.

Person Specification Qualifications
  • Registered Nurse, RGN/RN (Adult).
  • Diploma/degree in nursing or equivalent experience.
  • Nurse Prescriber.
  • Teaching experience.
  • Post registration Diabetes Qualification.
  • MSc in relevant topic.
  • Teaching qualification.
  • DESMOND or similar trained.
Training
  • Evidence of Continuing Professional Development with post registration training/experience in diabetes.
Skills
  • Car Driver who has access to a vehicle for work purposes.
  • Committed to improving services.
  • Able to work individually and as part of a multi-disciplinary team.
  • Flexible approach to meeting service & client needs.
  • Motivated and enthusiastic.
Information Technology
  • Computer literate, confident in word and e-mail.
Experience
  • Substantial post registration to include specialist diabetes experience.
  • Experience in developing and/or delivering teaching programmes.
  • Significant previous experience as a Diabetes Nurse.
  • Experience of working in community, primary care settings.
