Clinical Pharmacist - Admission and Discharge

An exciting opportunity has arisen for an enthusiastic and self-motivated pharmacist to join our admissions and discharge team at King's Mill Hospital.
This role includes working alongside the consultant-led Frailty Intervention Team (FIT), Integrated Discharge Assessment Team (IDAT), and the Short Stay Unit (SSU) medical and nursing teams to support medicines optimisation and timely discharge.
Our Pharmacy Department is committed to the training and development of all staff, and as part of this post, you will be given the opportunity to gain experience in the various domains of the RPS Advanced Practice Framework (APF) to develop the skills and knowledge required to progress your career further.
This role will be maximised by the use of independent prescribing, and you will be given the opportunity to gain this qualification if not already achieved. You will also help further develop the pharmacy service within this broad clinical area with opportunities to participate in a wide range of tasks.

Person Specification Knowledge- Clinical Pharmacy knowledge associated with a Pharmacy degree course.
- Experience gained through preregistration training.
- Experience gained through vocational practice.
- Experience of teaching pharmaceutical knowledge to other pharmacy staff.
- BSc, BPharm or MSc in Pharmacy. Member of the General Pharmaceutical Council
- Council Diploma or MSc in hospital clinical pharmacy gained through post-graduate study OR
- Independent prescribing - training to be provided in role if not already completed
- Clinical knowledge from aspects of career to date.
- Significant recent experience of hospital or community pharmacy.
- Strong interpersonal skills
- Good written and verbal communication skills
- Good organisational skills
- Evidence of commitment to CPD
- Evidence-based practice.
- Teaching experience
- Experience of extended roles
- Experience of working with multidisciplinary teams
- Experience of providing a specialist pharmacy service of some description
- Practice research.
- Flexibility in working hours
- Able to work extended hours and on call as needed.
