Clinical Pharmacist

Chiswick Medical Practice, serving the community across 3 sites Grove Park, Wellesley Road, and Chiswick Health Centre, is seeking a dedicated and experienced GP Clinical Pharmacist to enhance our multidisciplinary team. This role offers a unique opportunity to contribute to patient care by optimizing medication management and supporting our clinical services.
Main duties of the job- Patient Consultations: Conduct patient-facing clinics for chronic disease management, medication reviews, and provide expert advice on medication-related queries.
- Medicines Reconciliation: Ensure accurate and safe prescribing by reconciling medications post-hospital discharge and at other transition points.
- Prescribing Support: Assist GPs with prescription management, including addressing acute medication requests and repeat prescriptions.
- Quality Improvement: Lead initiatives to enhance prescribing practices, ensuring compliance with current guidelines and promoting cost-effective prescribing.
- Education and Training: Provide training to practice staff on medication management and support the development of protocols and guidelines.
- Collaborative Working: Work closely with GPs, nurses, and other healthcare professionals to deliver integrated patient care.

- Experience in a primary care setting, with a strong understanding of general practice operations.
- Excellent communication and interpersonal skills, with the ability to work effectively within a team.
- Registered pharmacist with the General Pharmaceutical Council (GPhC).
- Postgraduate qualification in clinical pharmacy or equivalent experience.
