Community Mental Health Practitioner - Memory Service West Suffolk

An exciting opportunity is available for an experienced Senior Mental Health Practitioner to join our Memory Assessment Service, based in Bury St Edmunds.

The Memory Assessment Service is clinician-led with a strong emphasis on team working to ensure that users of the service receive high quality, compassionate care throughout their memory assessment process. You will work within a dedicated team which includes nurses, including Independent Non Medical Prescribers, support worker, a Consultant Psychiatrist and which is well supported by admin.

The service is proud of being dynamic, responsive and proactive to the changing needs of the local population and its service users.

Main duties of the job

The successful candidate will join a service with a positive team culture, focused on learning, developing, improving and supporting each other as well as our service users. Staff wellbeing and development are key priority areas.

General responsibilities include:

1. Completing holistic assessments, including cognitive assessments using a validated assessment tool - training is provided as necessary.
2. Present assessments to the MDT to support the formulation of a diagnosis and follow-up plan.
3. Triage and Duty cover
4. Complete diagnostic discussions as appropriate, and complete onward referrals to appropriate agencies where indicated.
5. Participation in MDT meetings
6. Provide Management Supervision to colleagues as needed
7. Support Student Nurses on Placement, either as Practice Assessor or Supervisor, or to support others performing these roles.
8. Share ideas for service development and innovation in the Monthly team Business and Governance meetings.
9. Attending monthly Management and Clinical supervision.
10. Participation in research projects
11. Development of robust relationships with partner agencies and services.

You will be expected to be able to work autonomously and manage your time well.

You must be able to travel to various locations across Suffolk.

Person Specification Qualifications

• Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent

Experience

• Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience

Skills

• Able to effectively prioritise own workload

Knowledge

• Developing knowledge of local wider systems and networks
