Job Description

We are pleased to advertise this replacement post to support older people with frailty outside hospital beds. You will join a small team that performs home visits in geriatric medicine and provides clinical input & leadership to Ageing Well MDT Meetings in our primary care networks. We provide support to many teams already caring for the frailest in a variety of settings including care homes.

There is a lot happening in the care of frail people in the City and beyond. Community frailty virtual wards and a focus on falls and prevention are current major projects being commissioned. The Integrated Care Board is focusing attention on out of hospital and proactive care.

The right person for the job will have broad geriatric medicine skills and a CCT/CESR in geriatric medicine with an interest in community. This is a less than full time post and we welcome discussion about working in weekly or annualised patterns.

Main duties of the job

The job plan has been created to contain a total of 7 PAs, 5 clinical programmed activities and 2 SPA sessions. SPAs will include audit and clinical governance, teaching, appraisals, revalidation, CPD and research.

You will be an essential part of a multidisciplinary team responsible for delivering high quality care in Medicine for Older People in the community settings, ensuring patient care is based on need and ensuring services are accessible and acceptable to patients and carers.

Teaching and training of junior staff, medical students, nurses and other professionals allied to medicine are mandatory to your role and the development of the service.

You will actively participate in both departmental and organisational matters concerning Clinical Governance and Audit.

As part of your personal and professional development, you will take part in continuing professional development (CPD) and demonstrate commitment to revalidation.

Your peer support will cross our community team of community geriatricians, rehabilitation medicine doctors and also the acute geriatricians in the City. You will be part of a group of senior doctors including psychiatrists working for Livewell Southwest and all the usual revalidation and career support.

Person Specification Qualifications
  • Full GMC Registration, with a licence to Practice.
  • MRCP (UK) or equivalent.
  • One of CCT, CESR, European Community rights in Geriatric Medicine (or within 6 months).
  • Qualification in Teaching & Learning.
Experience
  • Secure background general medical experience and competency to manage on-call emergencies at senior level (if on call).
  • Experience of working with managers to improve patient experience and service delivery.
  • Experience in clinical audit.
  • Understanding of clinical audit.
  • Understanding of teaching principles for undergraduate, postgraduate and interdisciplinary learners.
  • Proven ability to motivate, support and work within a multi-disciplinary team.
  • High professional and ethical standards.
  • Excellent written and verbal communication skills.
  • A commitment to continuous professional development.
  • Basic computer/IT skills.
  • A special interest or advanced experience in community geriatrics and/or cognitive disorders.
  • Knowledge and understanding of the NHS.
  • Evidence of service development and enacting change.
  • Ability to lead clinical services.
  • Relevant published papers in peer reviewed journals.
  • Experience in teaching, particularly at a multi-professional level and or undergraduates.
  • Proven ability to organise and prioritise workload, delegating as appropriate and supervising staff.
  • Willingness to undertake additional professional responsibilities at local, regional or national levels.
  • Ability to balance competing pressures that come from holding more than one role.
