An exciting opportunity has arisen for a Continence Nurse to join our team at Homerton Healthcare NHS Foundation Trust. We are looking for a dynamic and enthusiastic practitioner with a particular interest in continence care. The service provides care to residents of the City and Hackney. You should be flexible and adaptable, an excellent communicator, enthusiastic, well-organised, and able to work well under pressure. The post holder will be required to work across sites including community sites and carry out visits to care homes and patients' homes. The successful applicant will have the ability to take on new challenges and actively participate in developments within the service.

The role is well developed within the trust and is supported by a Continence CNS. You will be expected to use initiative and innovations to develop and improve continence care. This role is suited to someone wishing to make a real difference to the quality of continence care and improve outcomes for patients using the services.

If you are an innovative, motivated, experienced nurse with excellent communication skills who is passionate about the promotion of continence, then we would like to hear from you. Consideration will be given to those who do not meet all the person specification but have a keen interest in continence care.

Main duties of the job

The post holder will assess and provide rehabilitation and treatment for patients who have a range of continence conditions.

They will provide expert clinical advice and be a resource for all disciplines of staff and Neighbourhood colleagues.

The post holder will undertake patient assessment to a high level including diagnosis, prescription, and monitoring of a treatment plan.

To act as an accountable role model through the provision of professional leadership and demonstration of competent practice.

Respond to urgent referrals for patients at risk of complications from incontinence, including e.g., pressure ulcer risk.

To contribute to the planning and development of the Continence service to improve the patient pathway.

Be involved in audit, research, and the teaching and education of other professionals.

Working for our organisation

In September 2021, the Adult Community Nursing Service launched its new structure. The new community nursing model was designed to encourage and support collaborative working with Neighbourhoods and other MDT colleagues to develop a more community-based model of care, working to reduce dependency on hospital-based care and supporting the delivery of personalised care and proactive care.

The Adult Community Nursing Service is made up of the Adult Continence Service, Tissue Viability and hub, Single Point of Access, and District Nursing Services. As a member of the service, you will be supported to set goals to meet your career aspirations. The well-being of its staff is a priority for the service and Trust, and there is a focus on promoting an open just culture and listening to feedback from staff and patients to ensure there is a focus on continuous service improvement.
