We are seeking a motivated, enthusiastic Mental Health Practitioner to join the South Manchester Mental Health Liaison Team, based at Wythenshawe and Trafford General Hospitals.
The Liaison Service has been operational since 2010, functioning 24/7 in line with CORE-24 standards.
Main duties- Provide clinical interventions as part of a multidisciplinary 24-hour mental health liaison team, focusing on relapse prevention, early discharge, and quality of life improvements.
- Assess patients in the A&E department, including under-16s in Pediatrics, performing risk assessments and management plans.
- Support ward and Intermediate Care assessments for individuals with complex needs, including organic and functional illnesses.
- Provide training to hospital staff on mental health recognition and appropriate actions.
- Assess, support, and care for individuals brought under section 136 of the Mental Health Act.

Shift patterns include Saturday (midnight to midnight), weekdays after 8 pm and before 6 am, and all hours on Sundays and public holidays.
Annual leave: 27 days plus bank holidays, increasing to 29 after 5 years and 33 after 10 years.
Person specification Qualifications- RMN, BA in Social Work, BSc in Occupational Therapy, or equivalent.
- Completion of Preceptorship or equivalent.
- Trained clinical supervisor.
- Qualifications related to physical health issues.
- Experience with diverse patient illness profiles and care settings.
- Proficiency in assessment tools for mental health conditions.
- Effective liaison with disciplines, agencies, and carers.
- Evidence of ongoing professional development.
- Experience in liaison services, A&E assessment, working with older adults and children, and psychosocial interventions.
- Understanding of Liaison Psychiatry, social factors in mental health, co-morbidities, Mental Health Act, Mental Capacity Act, and DOLS.
- Motivation to learn and develop.
- Risk assessment and management planning.
- Bio/psychosocial assessments.
- Discharge planning.
- Excellent communication skills.
- Time management and organizational skills.
- Flexibility, motivation, and teamworking abilities.
- Proficiency with IT systems such as Electronic Patient Records and Datix.
- Effective team member.
- Ability to work various shifts over 24 hours.
All positions are subject to pre-employment checks, including DBS. Enrolment in the DBS Update Service is required.
