We currently have an exciting opportunity in the Older Peoples' Mental Health team based at Llandough Hospital, working for Cardiff Social Services. This is a temporary position for an an initial 3 month contract with potential to extend. Pay rate for this role is up to £35 Umbrella company. We are looking for the right candidate to start asap. Access to own transport is essential. There is a blend of office and agile working in place.

The team work with with service users aged 65 years and over, who have mental health needs. The team is responsible for both assessing and reviewing those who require managed care and support.

In this role you will manage a caseload and also contribute towards the duty function, on a weekly basis which is office based. You will need to have strong assessment skills together with an ability to ensure regular reviews and risk management. Where intervention is required, you will be expected to provide a proportionate and timely response and effectively manage varying priorities. You will be required to undertake integrated assessments; mental capacity assessments, risk assessments and developing care and support plans with the aim of managing risk and finding appropriate solutions to meet needs / achieve outcomes. Ensuring that older adults are protected from neglect and abuse is also be a core component of the role. Emphasis is placed on team work and the role will entail working with various professionals.
