Primary Care Nurse (PCN) Clinical Service Lead

To build on established local partnerships and initiatives to transform our model of community mental health care and support for people living in the London borough of Southwark. The aim is to deliver a community mental health service with a specific focus on prevention and wellbeing to deliver integrated and personalised care closer to home.
In partnership with primary care colleagues, the service aims to provide early assessment, treatment, and improved access to evidence-based interventions for people with mental health needs. This will both avoid care being escalated into secondary care unnecessarily and will support people to step down out of complex care settings. Transfer back to primary care will also be a key element of this service.
Main duties of the jobClinical
- To provide the highest quality services within the resources allocated and provide specialist clinical advice to support people with mental health complex needs.
- To ensure that services are delivered within a robust clinical governance framework, monitoring and evaluating standards of care routinely in accordance with PCN governance arrangements.
- To ensure that the teams maintain clinical and care records in line with policies and procedures.
- To promote an ongoing process of service improvement within the team.
- To develop and maintain a high-performing team.
- To provide leadership within the team, supporting and developing the team to provide effective services.
- To ensure that the team works within the Policies of the PCN and be responsible for devising specific policies within the personality disorder service.
- To maintain effective team working with partners including primary care, statutory, voluntary, and independent sector organisations.
- To maintain regular links to the Mental Health Trust, particularly for ongoing support, supervision, training, and development; and support the design of a seamless patient pathway between primary and secondary care.
- To participate in service planning and development as part of the directorate business planning process.
South London and Maudsley NHS Foundation Trust (SLaM) provides the widest range of NHS mental health services in the UK as well as substance misuse services for people who are addicted to drugs and alcohol. We work closely with the Institute of Psychiatry, Psychology and Neuroscience (IoPPN), King's College London and are part of King's Health Partners Academic Health Sciences Centre. Job responsibilitiesThe post holder will lead a small team of nurses who will provide assessments and brief interventions in GP surgeries across Southwark. As an expert practitioner, you will provide teaching, coaching, and supervision to nursing staff in their delivery of nursing care.
The post holder will forge strong relationships with GPs, providing support and training to practice staff, in addition to supporting the step down of service users that have severe mental illness and who have achieved stability in their recovery.
Person Specification Qualifications- RMN or Dip/Bsc (Hons) in Mental Health Nursing OR Registered Occupational Therapist or Social Worker.
- Evidence of post-registration continuing professional development.
- Attended leadership/management training.
- Experience as a Senior Clinician in a Mental Health setting.
- Evidence of successful working within multi-disciplinary teams.
- Evidence of clinical supervision training and experience.
- Experience of working with a range of client groups and of using a variety of models and approaches to care.
- Experience of service user and carer involvement.
- Evidence of managing workload and ability to prioritise.
- Experience of racial awareness/diversity training.
- Experience training professionals in mental health, risk management, and access to services.
- Experience of participating in QI projects.
- Experience of working in Primary care.
- Experience of working with wider stakeholder groups.
- Able to demonstrate commitment to high quality care and service provision.
- Good interpersonal skills and the ability to foster good inter-team/inter-agency working relationships.
- Evidence of leadership ability, able to influence, involve, and develop individuals and teams.
- Decision making and problem-solving skills.
- Ability to use performance management information and demonstrate understanding of performance information.
- Excellent IT skills. Ability to develop and manage databases, develop reports, and presentations.
- Experience of managing feedback, investigation of complaints or similar.
