Job overview

This job will be suitable for candidates with experience in Neurosurgery at a trainee level ( 5 years) and ideally for those who have completed their neurosurgical training or are practicing neurosurgeons with a desire to obtain experience in expanded endoscopic endonasal and complementary open skull base work. It will be ideal for those with some prior experience of open skull base and basic endoscopic endonasal neurosurgery.

The successful applicant will be exposed to the full spectrum of endoscopic endonasal surgery with an emphasis on the expanded techniques. This will include relevant work in the sagittal and the coronal skull base with comprehensive exposure to expanded sellar and suprasellar approaches as well as the transpterygoid approaches. The surgical pathologies being treated using the expanded endoscopic endonasal and complementary open skull base approaches when applicable, will include pituitary (functional and non-functional adenomas) with an emphasis on complex adenomas; craniopharyngiomas; skull base meningiomas with a particular emphasis on tuberculum sella and planum meningiomas; chordoma and chondrosarcomas; sinonasal pathologies including esthesioneuroblastomas and adenocystic carcinomas; trigeminal schwannomas; other posterior fossa meningiomas and anterior cranio-cervical pathology. The fellow will also be exposed to complementary open anterolateral and posterior skull base approaches during the treatment of skull base and vascular pathologies.

Main duties of the job

A large majority of the surgical procedures are undertaken in conjunction with the ENT skull base surgeons and the fellow will therefore acquire an understanding of the reconstruction paradigm in endoscopic endonasal skull base surgery ranging from free grafts to pedicled nasoseptal flap and other advanced reconstruction techniques.

The fellow will provide assistance in theatre gradually assuming increasing responsibility under supervision; undertake a daily ward round and liaise with the team regarding management decisions. They will attend outpatients and the clinical meetings.

Specific to the skull base/pituitary service there will be: complex endoscopic endonasal clinic (once/month); open skull base/vascular clinic (once/month); joint pituitary clinic (twice/month); post-operative sinonasal clinic (twice/month) and fortnightly skull base/pituitary MDT. These clinics will highlight our paradigm pertinent to the preoperative work up and postoperative care of the patients.

In addition to above, there is a daily 7.30 a.m. combined clinical and radiology meeting; a weekly general Neuro-oncology tumour MDT, weekly epilepsy and movement disorders MDT, regular journal clubs and an academic teaching program.
