Job overviewWe are looking for an experienced, enthusiastic individual to join our Community Stroke Team. The team works with adults of all ages and is based on achieving client-centred goals in a time-orientated way, so experience in assessing and working with older people and with people with physical disabilities (stroke) is essential.
You will gain further experience in working with a range of other professionals and services. You will be joining a supportive group of colleagues, with regular in-service and professional forums, individual supervision, and a personal development plan that is regularly updated.
Main duties of the job- To deliver patient care based on principles of best practice, achievable through direct patient contact or via supervision.
- To carry out specialist, comprehensive assessments of patients with a complex presentation, using investigative and analytical skills to formulate individualised management and treatment plans.
- To provide specialised physiotherapy interventions at home or in a community setting as required, collaborating with colleagues in the community and secondary care health professionals to promote continuity of care.
- To identify own learning needs related to clinical knowledge and skills in the management of patients following stroke and with long-term neurological conditions.
- To support in the delivery of multidisciplinary education and training.
- To access further education programmes to develop knowledge and skills, and to share learning with the wider team.
- To support in the monitoring of the effectiveness of the community therapy service through the collation of data for ongoing audit.
- Contribute to the evaluation of research findings and disseminate them appropriately, including presentations at study days or conferences.
- To adhere to Whittington Health policy and procedures at all times.
- To be aware of one's own continuous professional development needs and maintain an annual personal development plan.

Detailed job description and main responsibilities- To provide high-quality effective physiotherapy assessment and treatment/management programmes for adults presenting with stroke in Haringey, who require a community-based service. To use highly specialist physiotherapy knowledge and skills to manage own complex caseload, as an autonomous practitioner.
- To act as a source of expertise on stroke, neurological conditions, and rehabilitation; providing advice to patients, carers, all grades of physiotherapists, other health care professionals, consultants, general practitioners, and social services.
- To support the use of evidence-based practice within the service, incorporating national guidelines e.g. stroke strategy into practice.
- Degree or Diploma in Physiotherapy
- Registration with the Health Professions Council
- Attendance on courses related to Neurological Physiotherapy e.g. Basic Bobath course
- Member of the CSP and relevant Clinical Interest group e.g. ACPIN
- Essential IT skills, including Word and PowerPoint
- Experience of running groups
- Experience of organising, writing, and delivering in-service training programmes
- Experience of involvement in clinical governance/service improvement initiatives
- Experience of taking physiotherapy students on clinical practice placements
- Experience of working with the voluntary sector
- Experience of supervising junior and non-qualified staff, including participation in personal development reviews
- Knowledge of current NHS priorities and policy
- Understanding of clinical governance activity and its application to physiotherapy services and practice.
- Knowledge of Physiotherapy professional issues.
- Knowledge of the stroke strategy, and the NSF for long-term conditions and related frameworks
- Knowledge of other medical conditions which you may encounter including, but not exclusively, respiratory, orthopaedics, and conditions of old age
- Evidence of attendance of post-graduate training related to the post
- Knowledge of the audit cycle and its uses.
