Job overview

Structured Clinical Management (SCM) is an evidence-based treatment for service-users with Severe Mental Health difficulties. The Croydon locality has adopted SCM as the framework for healthcare across the Directorate, embedding it in its operational policies. It is used with the full range of Secondary Care Patients including those with Psychosis Conditions, Mood and Affective Conditions, and Personality Disorder.

We are seeking to employ and support a mental health practitioner to take on a role as an SCM specialist within the Central CMHT. They will offer assessments and deliver high quality SCM treatment for the Secondary Care group, involving individual case management and low-medium intensity psychological interventions.

We are seeking an experienced Community Mental Health Practitioner to work within the Croydon Secondary Care Community Mental Health Pathway. Applications are welcome from qualified practitioners in Nursing, Social Work, Occupational Therapy, Psychology and Psychotherapy including Mental Health and Wellbeing Practitioners and Clinical Associates in Psychology, Medicine and Pharmacy.

As part of the National Community Transformation Programme, this post holder will be supported with training and supervision in the Structured Clinical Management model for Case management.

Main duties of the job

Croydon Community Mental Health Teams (CMHTs) have undergone service transformation aimed at enhancing service delivery quality, improving user experiences, and maintaining staff morale. The new teams work cross-diagnostically with individuals from a locality who have mental health needs requiring support from Secondary Care.

Detailed job description and main responsibilities

The post holder will receive support around the delivery of low-medium psychological interventions under supervision as part of their delivery of the SCM model of care. They will act as a sub-specialist in the team for the SCM model and will hold a weighted caseload to reflect the demands of the role.

Working alongside other members of the multi-disciplinary team, the post holder will work closely with care coordinators and case managers in identifying barriers to discharge or implementing care plans. They will collaborate with colleagues, primary care, and the patient's family/carers to facilitate discharge and ensure the estimated discharge date is progressed.

The post holder will work a normal shift pattern of Monday- Friday 9:00-17:00 and will need excellent clinical and risk assessment skills to carry out comprehensive patient assessments while ensuring effective transitions to other appropriate clinical pathways.

Experience in providing a boundaried approach to complex patients, including those with personality disorders, is advantageous. Delivery of low and medium psychological interventions is also an asset.

The post holder will work as an experienced mental health professional providing care for service users in their recovery journey from severe and enduring mental health challenges.

Person specification Qualifications
  • Relevant professional qualification - Mental Health Nursing, Social Work, Occupational Therapy with affiliated professional registration (NMC, HCPC, BPS Registration).
  • Mentorship qualification/assessor (Nursing and Social Work).
  • Relevant continuous in-house or external training.
  • Training in psychosocial model.
  • Training in SCM.
Experience
  • Experience of interagency working.
  • Experience of managing a caseload.
  • Experience of working with individuals experiencing severe and enduring illness.
  • Experience of working with diverse groups of patients and staff.
  • Experience of working in the community.
  • Experience of supporting practice development in clinical areas.

Skills and Abilities
  • Knowledge of CPA, Community Care Act, Carers Act, Mental Health Act and other relevant legislation.

  • Very good understanding of care plans and holistic care.
  • Problem-solving skills.
  • Skilled in supervision of others and providing critical feedback.
