Job Description

Our Transfer of Care Team is dedicated to managing patient discharges from acute and community hospitals across Cambridgeshire. We pride ourselves on a Home First approach, ensuring individuals can stay within their own communities while receiving the care and support they need.

As the Transfer of Care Team Manager, you will oversee the operational flow across both our North and South teams. You will ensure that discharge processes are managed efficiently and in line with timescales, while also leading initiatives such as Discharge to Assess. You will be at the heart of our commitment to providing high-quality, person-centred care.

The key responsibilities include leading, managing, and supporting the North and South Transfer of Care Teams. Additionally, you will oversee discharge processes to ensure compliance with timescales and maintain a safe, efficient patient flow. Collaboration with health professionals, community services, and families is crucial to coordinate effective discharge plans.

You will manage and resolve any operational challenges related to discharge, ensuring continuity of care. Monitoring team performance and providing feedback and development opportunities are essential parts of the role. You will contribute to the ongoing development and improvement of team processes and individual care services.

We are looking for an exceptional Team Manager to join us on our journey.

The Transfer of Care Teams operate within specific office hours, but that does not mean we will not consider flexible working opportunities. Our staff receive a salary enhancement between 7% and 15% depending on their selected working pattern.

  1. 7% for 1 weekend day a month and 1 Bank Holiday a year
  2. 8% for 1 weekend day a month and 2 Bank Holidays a year
  3. 10% for 1 weekend day a month and 3 Bank Holidays a year
  4. 15% for 2 weekend days a month and 3 Bank Holidays a year

Interviews are planned for the week commencing the 17th of March.

What will you be doing?

Day-to-day responsibilities of a Team Manager in Assessment include:

You will supervise and develop practitioners, identifying their strengths and areas for improvement, and implement appropriate plans to achieve this. You will manage the delivery of the team to ensure it is a high quality and effective service that responds to the needs of adults and their families or carers in a timely manner within statutory guidelines or service specifications. Additionally, you will manage the planning, allocation, and review of cases to ensure priorities are effectively managed.

Leading on customer service and handling stage 1 complaints will also be part of your responsibilities. You will manage the recruitment and staffing of the team, provide expert knowledge of safeguarding processes, policy, and case law, and lead on initiating, developing, and sustaining internal and external working relationships with other professionals and partner agencies.

You must:

  • Be a qualified Social Worker with a Social Work England registration
  • Have the ability to work from a variety of locations and travel countywide independently
