Social Worker - Complex Care Team (Westminster City Council)

Location: Victoria, London (Hybrid Working)
Rate: £32 per hour (Umbrella)

About the Role:
Westminster City Council is seeking an experienced and dedicated Social Worker to join the South Complex Care Team. This role involves a mix of office-based and remote working, requiring regular attendance at the office alongside flexibility to work from home (as agreed with the manager).

You will be responsible for conducting Care Act assessments, risk assessments, safeguarding enquiries, Continuing Healthcare (CHC) assessments, and Mental Capacity Assessments (MCA). The role also involves managing referrals, placements, and case reviews.

Key Responsibilities:

1. Care Act assessments, care planning, and risk assessments
2. Lead safeguarding enquiries as a Lead Enquiry Officer
3. Conduct Continuing Healthcare (CHC) & Mental Capacity Assessments (MCA)
4. Duty work, including occasional visits & casework for urgent needs
5. Work collaboratively with a multi-disciplinary team (MDT)
6. Utilize MOSAIC/FWI systems for care management & reporting
7. Support individuals in gaining independence through strength-based approaches

What We're Looking For:

1. Social Work England registered
2. Strong Local Authority experience in Complex Care
3. Knowledge of Care Act 2014, CHC, MCA, and safeguarding
4. Ability to work independently & as part of a team
5. Strong report writing skills & proficiency in MS Word, Outlook, Teams, and Office applications
6. Experience using MOSAIC/FWI systems is highly desirable
7. Commitment to delivering high-quality casework & promoting independence
