Hanham Health is looking for a GP to join our practice working 5 sessions across our surgeries in Bristol.
With demands on General Practice being greater than ever, Hanham Healthhas implemented a system which puts staff welfareat the centre of BMA Safe Working in General Practice guidance.

Main duties of the jobIn accordance with the practice rota, toundertake a variety of duties to include:
- Surgery consultations and queries includingface to face, telephone, video and online consulting.
- Visiting patients at home.
- Duty doctor and emergency on-call coverduring opening hours.
- Checking and signing of prescriptions anddealing with associated queries.
- Dealing with workflow in a timely manner.
We are a 9 partner, 32,500 patient practice operating across 4 sites inHanham, Oldland, Pilning and Almondsbury, on the northand eastern edges ofBristol, with opportunities to work at any of these sites.

Job responsibilitiesMake professional,autonomous decisions in relation to presenting problems including historytaking, examination, investigation, diagnosis, treatment and referral.
Work with clinical and operational colleagues in delivering a safe,high performing, effective, sustainable, innovative person-centred medicalservice.
Undertake routine surgery consultations, home visits where necessary,online and telephone queries from patients and health care professionals.
Undertake on-call duties as part of the on-call rota, where agreed.This may include requesting that patients present at the surgery, telephone andonline consultations, home visits and triage.
Contribute to thedevelopment of and adhere to protocols and care plans for the management ofchronic disease and common medical conditions.
Contribute towardsmeeting the indicators outlined in the QOF and other locally and nationallyagreed guidance.
Provide appropriatepreventative healthcare, counselling, education and advice.
Adhere to thepractices prescribing formulary.
Provide freecertification in line with Schedule 9 of NHS GMS Regulations (1992).
Attend practice andclinical meetings and forums.
Contribute to thesuccessful implementation of continuous improvement and quality initiativeswithin the practice.
Record clear and accurateconsultation notes to agreed standards and maintain patients clinical records.
Participate inclinical governance activity and contribute to the improvement inequality ofhealth outcomes through the practices audit programme
Perform clinicallyrelated administrative and non-clinical duties necessary for the delivery ofPersonal Medical Services.
Engage inappropriate educational activity in order to maintain clinical competence and performance,including participation in the annual GP appraisal scheme.
Support the wider practiceteam and respond to requests for advice and assistance.
Person Specification Experience- Using electronic medical records.
- Working autonomously and as part of a team.
- QOF and clinical audit.
- Understanding of clinical risk management and clinical governance.
- Use of EMIS.
- Interest in Mental Health.
- Interest in Care of Elderly.
- Coil and implant fitting/family planning.
- Full registration with GMC.
- Inclusion on the GP Performers List.
- Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent.
- MRCGP.
- Specialist interest /development / training.
