About this role

We are looking for a Support Worker to join the City Mobile Intervention Support team.

The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

Thames Reach welcomes applications from people with transferable skills.

You will:

  • Assess the support needs of new clients in a person-centred manner, using a standardised process.
  • Manage a caseload of clients, identifying the most appropriate route towards sustainable and permanent accommodation, and devise plans to achieve this.
  • Work alongside various partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
  • Represent the team at multi-agency meetings, such as Task & Targeting meetings.
  • Work with clients who identify as women and develop specialist knowledge in women's homelessness.
  • Accurately record all work done.

To succeed as the Support Worker you will have:

  • Have strong communication skills with the ability to form and maintain effective working relationships with all partners.
  • Have commitment to supporting people to achieve the best outcomes as they work towards independence.
  • Have the capability to carry out administrative tasks alongside practical support work, and manage and mitigate risk.
  • Have the ability to work autonomously as well as part a team.
  • Have the commitment to working in a challenging environment with the ability to manage conflicting priorities and deadlines.
  • Have IT skills with the ability use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone.
