Job Description

Our heart failure specialist nurse team plays a pivotal and dynamic role in the care of our patients with heart failure. We have developed excellent systems of care, for example automated NTproBNP referrals and a heart failure clinical pathway, and are integrated with an excellent multi-disciplinary team including the community and palliative care teams. You will be working in a team caring for acute inpatients primarily, from first diagnosis to end stage heart failure. Your role will be focused on ensuring medicines are optimised, providing lifestyle and self-care advice to patients and carers, and ensuring the patient has a comprehensive management plan on discharge. You will be preparing patient information for medicine and device MDTs and inputting data into the national heart failure database (NICOR).

You will receive a comprehensive induction programme, will be supported by band 7 HFSN, and follow the BSH competency framework to guide development in your role.

Main duties of the job

The post holder will facilitate the provision of high quality specialist care to patients with acute and chronic heart failure assisting in the diagnosis, treatment and follow up care in the outpatient setting.

They will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the specialty.

Education & professional Qualifications
  • Evidence of post registration qualification relevant to specialty
  • Advanced Life Support provider
  • Educated to Master's degree or willing to work towards
  • Mentorship qualification or ENB 998, or other teaching qualification or evidence of relevant experience
Experience
  • Evidence of teaching and developing others
  • Completed Non-Medical Prescriber Course or willing to undertake.
  • Completed Clinical Assessment Course or willing to undertake.
  • Recent experience of nursing cardiology patients in an acute setting.
  • Substantial nursing experience at a senior level.
  • Experience of running nurse led clinics
  • Experience of managing own caseload of patients
