Greater Middlesbrough PCN is looking for a dynamic pharmacist to join our highly skilled and experienced team of primary care clinicians and medicines management support staff. Main Duties of the Job

The role offers a varied work agenda providing medicines optimisation support and expert pharmaceutical advice across the 2 practices in our network, including the implementation and delivery of prescribing initiatives, improving prescribing safety, and provision of medication reviews, fully supported by an experienced network of colleagues. The successful applicant should be able to demonstrate excellent clinical and communication skills and have experience of implementing medicines optimisation, quality and patient safety initiatives, and resolving prescribing issues. You will be a Registered Pharmacist with the General Pharmaceutical Council with a minimum of 3 years post-qualification experience, who is preferably an Independent Prescriber. You must also be computer literate, organised, and able to work independently as well as part of a team. The successful candidate will be able to prioritise workload, build key relationships, and be confident interacting with a wide range of stakeholders such as GPs, Nurses, secondary care clinicians, and patients.

Greater Middlesbrough PCN consists of multidisciplinary teams that currently include Clinical Care Coordinators, Care Coordinators, Clinical Pharmacists, Mental Health Practitioners, First Contact Physio, and Social Prescribing Link Workers.

The role will include working across all the PCN providing additional flexibility by adding to the growing expansion of the primary care workforce.

Job Responsibilities

Primary Duties and Areas of Responsibility

  1. Expected to work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
  2. Be a prescriber, and work with and alongside the general practice team.
  3. Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD, and asthma), and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).
  4. Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities.
  5. Provide leadership on person-centred medicines optimisation and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.
  6. Deliver a programme of structured medication reviews to support patients to take their medications to get the best from them, reduce waste, and promote self-care.
  7. Undertake a leadership role in supporting further integration of general practice with the wider healthcare teams to help improve patient outcomes, ensure better access to healthcare, and help manage general practice workload.
  8. Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.
  9. Undertake a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, and community pharmacists.
  10. Be part of a professional clinical network and undertake appropriate clinical supervision.
