Job Description
Doctors Direct have an exciting opportunity for a Speciality Doctor , General Adult psychiatry to work with us at Islington in Complex depression anxiety & trauma team (CDAT).
We are looking for a doctor dedicated to, and passionate about, improving health and wellbeing within the North Islington Community.
Your key responsibilities will be to:
The duties of the post are based on the headings of the GMC's Good Medical Practice. Providing a good standard of practice and care
Undertake clinical assessment of patients with mental health problems
You will have medical responsibility for patients in CDAT. You will conduct full psychiatric assessments either at the team base or in patients' homes. Following an assessment, you will formulate management plans. You will carry out risk assessments on all new cases and ensure risks are addressed in management plans. You will record clinical information, including all diagnoses, medication, risk assessments, DIALOG+ rating scales and DIALOG+ care plans and outcome all appointments using the Trust's electronic patient records system RIO, and update patients' records regularly. You will discuss your assessment and management plan with your consultant. You will send the GP and client a copy of your assessment within a week of assessment.
There are weekly team meetings, and you will take an active role within the Multidisciplinary Team at all stages of managing patients. This includes urgent assessments, joint reviews with team members and CPA reviews. Much of the work is multi-disciplinary, so you will have the opportunity to work with all members of the team. You will dictate or type letters to GPs after each patient contact or change in management plan to keep them informed and record changes in management on RIO.
If medical students or junior trainees are attached to the team, you will facilitate their learning by including them in assessments or clinical reviews (with the patient's permission) and discussing the assessment with them.
You will familiarise yourself with the Mental Health Act and Mental Capacity Act and work within this legislation. You will adhere to Trust Policies.
You will maintain accurate, legible, contemporaneous and useful clinical records for all patients in whose care you are involved.)
