Responsibilities:

• Conduct assessments and reviews under the Care Act 2014, ensuring tailored support plans for service users.
• Connect individuals with universal and voluntary services to promote independence.
• Support colleagues with complex cases, including mental capacity and best interest assessments.
• Work across designated services within the team to ensure high-quality outcomes for adults in need.

To be considered for the social work role, ideally you will have:

• 1-3 years in a similar adult social work role.
• Diploma/Degree in Social Work and Social Work England registration.
• Confidence in assessments, support planning, and a commitment to enhancing quality of life.
