Learning Disabilities & Autism Service - Supporting Independence and Well-Being
Our Learning Disabilities & Autism Service is dedicated to supporting the independence and well-being of adults and young people with learning disabilities and autism.
We are seeking a Transition Link Social Worker to support young people aged 14-18 with complex needs as they transition from children to adult services.
Within our community team and the Moving on Service, we provide social care support that empowers individuals, promoting choice and independence in their lives. Our passionate Social Workers collaborate with health professionals to deliver person-centred care that supports adults with learning disabilities to achieve their potential and maintain independence in the community.
As a Transition Link Worker, you will be the bridge in planning each young person's long-term future in collaboration with them, their families and a network of professionals.
As a qualified Social Worker, you will bring experience with health, education and social care for young people with additional needs, along with knowledge of relevant legislation like the Care Act 2014. Strong communication skills are essential, as you'll build collaborative relationships with young people, carers and stakeholders across various services. This role requires a self-starter who is organised, capable of clear and accurate reporting and passionate about delivering high-quality, customer-focused support.

This position comes with a 5% market supplement payment. All market supplement payments are subject to review on an annual basis.
