Job Description

We have an exciting opportunity for a band 5 Registered nurse to join the community-based Nurse led Anticoagulation Service. The city wide nurse led anticoagulation service undertakes the effective delivery of nurse-led anticoagulation therapy to patients within the community setting utilising near patient testing. The role involves the successful candidate undertaking nurse-led clinics, undertaking home visits for the housebound in addition to the management of self testing/remote monitoring patients.

The successful applicant will need to be flexible, used to working under pressure, and have excellent interpersonal and communication skills. You will need to have an adaptable approach to work in order to respond to the dynamic changes that can occur within the service on a day to day basis.

Main duties of the job

The successful candidate will be proactive and willing to assist with future developments within the service. They will work as part of a skilled team and will be responsible for patient assessment, care planning and effective care delivery. Anticoagulation knowledge would be an advantage, however full training will be given.

A flexible approach to duties is essential. We provide a 7 day service, with working hours between 08:00 - 17:00 Monday to Friday, and 09:00 - 13:00 on a Saturday, Sunday and bank holidays. There is an expectation that the successful candidate will work weekends and bank holidays.

The ability to commute within CHCP CIC's geographical areas with access to appropriate means of transport is essential - a full UK driver's license is required.

Person Specification Qualifications
  • Degree in Nursing Studies
  • First level Registered Nurse or working towards final assessment to achieve first level registration
Knowledge
  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Demonstrate an understanding of evidence-based practice
  • Demonstrate an understanding of national guidelines relevant to area of specialism.
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process
  • Able to mentor students
  • Evidence of professional development
  • Have a working knowledge and understanding of Anticoagulation Therapy
Experience
