Job Description

We have an awesome opportunity for an Experienced Associate Clinical Case Manager for Children and Young People (Self Employed) to join us.

Working Remotely covering East Anglia and London locations.

Anglia Case Management is recognised in the field as one of the most established providers of specialist case management services to adults and children who have incurred acquired brain injury or other complex neurological conditions. We operate in the independent sector and the company is part of the Active Care Group.

What you'll be working:

  • Full time or Part time - with minimum 3 days a week availability for case management
  • Remote working to cover East Anglia & London
  • Reporting to Clinical Director

This is a self-employed position, minimum 3 days per week.

Remote and flexible working. Predominantly weekday work during daytime hours, but there will occasionally be a need for flexibility to work outside the normal 9.00am to 5.00pm routine, to meet individual client needs.

Rates of pay:

Professional Time: £45 - £65/hour depending on experience

Travel time: £30/hr

Mileage: £0.45/mile

What you'll be doing:

Some of the Main Roles & Responsibilities:

  • Providing case management services to a small client base of children with a range of complex disabilities.
  • The case load may include those with acquired and traumatic brain injury, spinal cord injury, birth injury, amputation, and other complex conditions.
  • Undertaking assessments of clients' needs and setting goals for intervention in conjunction with clients and their families where appropriate.
  • Providing regular updated reports to relevant parties within ACM standards and guidelines where required.
  • Ensuring the clients' well-being and safety is protected at all times.
  • Working with our case management support team to design bespoke recruitments, induct and train care staff employed on behalf of the client.
  • Supporting the client with medical assessments in relation to benefits.
  • Advising on specialist equipment, additional living needs, and housing requirements.
  • Assisting with holiday plans and costs, where this is required because of the client's complex needs.

What you'll have:

Experience as a Case Manager working with Children & Young People would be ideal.

  • Relevant qualification in nursing, social work, occupational therapy, speech & language therapy, or physiotherapy.
  • Current registration with the appropriate professional body (e.g., SWE, HCPC, NMC).
  • Minimum 3 years post-qualification experience (pay and induction tailored accordingly).
  • Experience of working with clients who have Cerebral Palsy.
  • Experience of working with clients who have brain injury, neurological, and other complex conditions.
  • Knowledge/skills in support planning, risk assessment, and management plans.
  • Highly effective verbal and written communication skills; strong problem-solving skills and the ability to manage difficult and at times volatile situations.
  • Confidence to liaise with clients, their families, support workers, legal teams, and other professional staff.

Client Base:

Anglia Case Management provides case management to both adults and children and young people. Most of our CYP referrals are either children with cerebral palsy or traumatic brain injury. The majority of our adult clients have suffered an acquired or traumatic brain injury, and some have other associated injuries, which compound their needs. We also take referrals for clients with other complex conditions, such as multiple MSK injuries, amputees, or spinal cord injuries. We have clients who we are supporting at work or at home, and others who have profound needs requiring 24-hour support.
